Dear Mr. Scott:
The Division of Environmental Management has completed a review of the "as -built"
plans submitted July 24, 1989, and finds that a permit amendment is not necessary.
The following items were reviewed and found to be acceptable minor changes to the
approved plans for the subject permit.
1. The top elevation and the ground level elevation of the pump station wet well are
now both shown six inches higher.
2. The top rim of the effluent manhole is shown three feet lower, making this manhole
three feet shorter. This should not effect the effluent discharge.
3. The influent box is slightly redesigned and relocated to the other side of the cat
walk, a few feet from where it was. The redesign should not effect the original
function or capacity.
4. The foundation for the tertiary filter is two feet longer, and should provide better
support.
5. The WWTP six inch drain line is now shown running counter clockwise around the
plant to reach the pump station wet well; where before the six inch line was shown
clockwise around the plant to reach the wet well.

Dear Mr. Scott:
A letter of request for an Authori.7.4tion to Construct was received September 23,
lg873 by the Division and final plans and specifications for the subject project have
been reviewsd and found to be satisfactory. Authorization is hereby granted for the
construction of a 0.23 MCD wastewater treatment facility consisting of a 40,000 gallon
aerated surge tank with 160 GPM duplex pumps, a 287,000 gallon aeration tank, a 47,920
gallon secondary clarifier, a 23,000 gallon aerated sludge holding tank, three (3) at
540 CFM each rotary blowers, a 174 square feet of total area traveling bridge type
tertiary filter system consisting of 29 filter cells and 120 GPL1 duplex backwash
pumps, a 4800 gallon chlorine contact tank with a v-notch dual gaseous chlorinator a
flow recorder and totalizer, a 2046 gallon pest aeration tank, 215 linear feet of.
12-inch gravity discharge pipe and all related valves, pipes, and appurtenances to
serve Bradfield Farms Subdivision Phase I"'locsted in Cabarrus County.
This Authorization to Construct is issued in accordance with Part HI paragraph B
of the NPDES Permit No. NCO064734 issued September 10, 1986, and shall be subject to
revocation unless the wastewater treatment facilities are constructed in accordance
with the conditions and limitations specified in Permit No. NCO064734.
The Permittee must employ a certified wastewater operator in accordance with Part
III, paragraph C of the referenced permit.
The sludge generated from these treatment facilities must be disposed of in
accordance with G.S. 143-215.1 and in a manner approvable by the North Carolina
Division of Environmental. Management.

The Mooresville Regional Office, telephone number 704/663-1699 shall be notified
at least twenty-four (24) hours in advance of the installed system so that an in -place
inspection can be made. Such notification to the Regional Supervisor shall be made
during the normal office hours from 8:00 a.m. until 5:00 p.m. on Monday through
Friday, excluding State Holidays.
In event the facilities fail to perform satisfactorily in meeting its NPDES
permit effluent limits, the Permittee shall take such immediate corrective action as
may be required by this Division, including the construction of additional wastewater
treatment and disposal facilities.
The sand media of the traveling bridge type filter system must comply with the
Division's sand specifications and must be analyzed and approved by this Division
either by direct sampling or by acquisition of filter sand from a dealer who is
currently certified by the Division as an acceptable source.
A copy of the approved plans and specifications shall be maintained on file by
the permittee for five years from the date of issuance.
Upon completion of construction and prior to operation of this permitted
facility, a certification must be received from a professional engineer certifying
that the permitted facility has been installed in accordance with the approved plans
and specifications. Mail the certification to the Permits and Engineering Unit, P. 0.
