Enclosed please find a copy of the Compliance Evaluation Inspection Report for the inspection
we conducted at the Project/Site on Date. Please note that in the stormwater inspection report
summary we ask that you submit an updated Stormwater Pollution Prevention Plan (SPPP) and
analytical and qualitative monitoring reports to the Winston-Salem Regional Office by July 7,
2023.
The report should be self-explanatory; however, should you have any questions, please do

Inspection Summary:
The Stormwater Pollution Prevention Plan (SWPPP) and related documentation was incomplete for employee training,
facility inspections, and discharge monitoring reports. Analytical and qualitative monitoring has not been conducted since
2020. The facility had an abundance of cut wood particles like mulch, bark, and wood chips especially at the composting
area. There was a sediment basin with baffles, but it was full of sediment. And the flow to the front of the facility appears to
be sheet flow. Attention should be given to good housekeeping and outlet maintenance. An updated Stormwater Pollution
Prevention Plan and qualitative and analytical monitoring reports must be submitted to the Winton -Salem Regional office by
