"As Built" Certification
Dear Mr. McIntire;
In reference to the above noted waste water treatment plant, and the permit
requirement for an Engineer's "Certification of Construction Compliance" I have made the
following investigations.
1.) Reviewed a plan prepared by Mr. Robert E. Etue,
2.) Reviewed the design plan as submitted, and approved by your office,
3.) Visited the site for my own inspection, and
4.) Met with the WWTP Operators (Mid South)
The Etue plan is an "As Built" survey which has been certified as accurate by Mr.
Etue; who is a Registered Professional Engineer in North Carolina.
With the "As Built" certification as a reference I did not re -measure the structures;
but rather used my field visit to visually verify the general configuration of the components
and operability of the system. I have also been informed that the treatment plant operators
( Mid South Water Systems, Inc. ) have reviewed the system, and performed start-up tests
on the system's mechanical components. The Mid South operators, while noting some
details that they wish to have changed, are prepared to accept the plant for operation.
My research and observation indicate to me that the constructed system; due to
field encountered complexities; has been constructed differently from the approved plan.
The system, however, has the same components and the same treatment process as that
which was approved by your office. I believe that the system; as constructed; is
substantially in compliance with the original "approved" design plans.
I am hereby certifying that this WWTP has been constructed in an appropriate
manor and is basically ready for operation.

any questions, please give me a call.

Authorization to Construct Ammendment
Country Woods Subdivision
Union County
The Division issued an Authorization to Construct (ATC) to Mid South Water Systems July 25, 1997 for the above
referenced facility. The ATC authorized construction of a 160,000 gpd wastewater treatment system. This ammendment to
that ATC authorizes construction of a 480,000 gpd wastewater treatment facility consisting of three (3) 160,000 gpd
treatment trains as descirbed in the July 25, 1997 ATC.
This Authoriztion to Construct requires that all components of the above described wastewater treatment facility be
at least 50 feet from the facility's property line. Also, as a condition of this Authorization to Construct, in addition to the
berm specified on the plans a wood fence and vegetative buffer shall be constructed and should be verified by the engineer prior
to submission of the Engineering Certification.
This Authorization to Construct is issued in accordance with Part III, Paragraph A of NPDES Permit No.
NCO065684 issued November 14, 1994, and shall be subject to revocation unless the wastewater treatment facilities are
constructed in accordance with the conditions and limitations specified in Permit No. NC0065684.
The sludge generated from these treatment facilities must be disposed of in accordance with G.S. 143-215.1 and in a
manner approved by the North Carolina Division of Water Quality.
In the event that the facilities fail to perform satisfactorily, including the creation of nuisance conditions, the
Permittee shall take immediate corrective action, including those as may be required by this Division, such as the
construction of additional or replacement wastewater treatment or disposal facilities.
The Mooresville Regional Office, telephone number (704) 663-1699, shall be notified at least forty-eight (48) hours
in advance of operation of the installed facilities so that an in -place inspection can be made. Such notification to the regional
supervisor shall be made during the normal office hours from 8:00 a.m. until 5:00 p.m. on Monday through Friday,
excluding State Holidays.
Upon completion of construction and prior to operation of this permitted facility, a certification must be received
from a professional engineer certifying that the permitted facility has been installed in accordance with the NPDES Permit,
this Authorization to Construct and the approved plans and specifications. Mail the Certification to the Permits and
Engineering Unit, P.O. Box 29535, Raleigh, NC 27626-0535.

Upon classification of the facility by the Certification Commission, the Permittee shall employ a certified
wastewater treatment plant operator to be in responsible charge (ORC) of the wastewater treatment facilities. The operator
must hold a certificate of the type and grade at least equivalent to or greater than the classification assigned to the wastewater
treatment facilities by the Certification Commission. The Permittee must also employ a certified back-up operator of the
appropriate type and grade to comply with the conditions of Title 15A, Chapter 8A, .0202. The ORC of the facility must
visit each Class I facility at least weekly and each Class II, III, and IV facility at least daily, excluding weekends and holidays,
and must properly manage and document daily operation and maintenance of the facility and must comply with all other
conditions of Title 15A, Chapter 8A, .0202. Once the facility is classified, the Permittee must submit a letter to the
Certification Commission which designates the operator in responsible charge within thirty days after the wastewater
treatment facilities are 50% complete.
A copy of the approved plans and specifications shall be maintained on file by the Permittee for the life of the
facility.
The Operational Agreement between the Permittee and the Environmental Management Commission is incorporated
herein by reference and is a condition of this Permit. Noncompliance with the terms of the Operational Agreement shall
subject the Permittee to all sanctions provided by G. S. 143-215.6 for violation of or failure to act in accordance with the
terms and conditions of this Permit.
Failure to abide by the requirements contained in this Authorization to Construct may subject the Permittee to an
enforcement action by the Division of Water Quality in accordance with North Carolina General Statute 143-215.6A to 143-
215.6C.
The issuance of this Authorization to Construct does not preclude the Permittee from complying with any and all
statutes, rules, regulations, or ordinances which may be imposed by other government agencies (local, state, and federal)
which have jurisdiction.

A letter of request for an Authorization to Construct was received January 17, 1997 by the Division and final plans
and specifications for the subject project have been reviewed and found to be satisfactory. Authorization is hereby granted for
the construction of a 160,000 grid wastewater treatment system consisting of a 40,000 gallon flow equalization tank, a
160,000 gallon diffused aeration basin, a 26,667 gallon secondary clarifier, a 32,000 gallon aerated sludge holding tank, four
tertiary filter cells with an integrated clearwell, mudwell and filtrate holding chamber, a 3,333 gallon chlorine contact
chamber, a 1,500 gallon dechlorination chamber, a recording type flowmeter, an emergency power diesel generator and all
blowers, piping and appurtenances with final discharge of treated effluent into Goose Creek in the Yadkin -Pee Dee River
Basin.
This Authoriztion to Construct requires that all components of the above described wastewater treatment facility be

treatment facilities: The proposed addition to the
existing plant includes a 40,000 gallon flow equalization
tank, dual 50,000 gpd extended aeration package plants,
a 190,000 gpd effluent filtration unit, two 1250 gallon
chlorine contact tanks, a 2064 gallon post aeration tank,
and dechlorination.

PART IV - EVALUATION AND RECOMMENDATIONS
The wastewater treatment facility serving Country Woods II was
in good operating condition at the time of the site inspection.
The existing facilities have a design capacity of 0.09 MGD.
An authorization to construct has been issued and the construction
is underway for an additional 0.10 MGD for a total of 0.190 MGD.
This office recommends that no dwellings be located closer than 150
feet to the wastewater facility.

No adverse impact was noted on the receiving stream as a
result of discharge. Several other wastewater treatment plants are
located on Goose Creek or its tributaries.
It is recommended that the subject permit be renewed as requested.
