Stewart of Land Design Engineering Services, Inc., the Division of Environmental Management acknowledges the specified design modification to the Country Woods II - Phase II wastewater treatment facilities. According to Mr. Stewart's letter, the post aeration tank for the new plant was constructed on the south side of the plant rather than the north side (as originally specified). Dear Mr. Weber: `-} A letter of request for Authorizatior, to Construct was received October 13, 1988, by the Division of Environmental Management; however, it has been determined by the Division's Mooresville Regional Office that the construction of these facilities was completed prior to the issuance of approval from this Division. The submitted plans and specifications for the subject project have been reviewed and found to be satisfactory. The Division hereby grants approval for the operation of a 0.045 MGD wastewater treatment facility consisting of an existing flow splitter, a bar screen, two (2) aeration tanks with a total volume of 49,000 gallons, a 7484 gallon capacity clarifier, a 7500 gallon capacity sludge holding tank, two (2) blowers capable of delivering 185 CFM each, a 1524 gallon capacity chlorine contact chamber equipped with a tablet chlorinator, an effluent flow meter with totalizer, a 1500 gallon capacity post aeration tank, an existing outfall, and all associated valves, piping, and appurtenances to serve Country Woods II Subdivision, Phase II. This Authorization to Operate is issued in accordance with NPDES Permit No. NCO065684 issued April 4, 1988, and shall be subject to revocation unless the wastewater treatment facilities are constructed in accordance with the conditions and limitations specified in Permit No. NCO065684. The issuance of this Authorization to Operate does not relieve Mid South Water Systems, Inc. of any liability or enforcement actions which the Division may take as a result of constructing these facilities prior to the issuance of an Authorization to Construct. The Permittee must employ a certified wastewater operator in accordance with Part III paragraph C of the referenced permit. The Mooresville Regional Office, phone no. 704/663-1699 shall be notified at least twenty-four (24) hours in advance of operation of the installed facilities so that an in -place inspection can be made. Such notification to the regional supervisor shall be made during the normal office hours from 8:00 a.m. until 5:00 p.m. on Monday through Friday, excluding State Holidays. In the event the facilities fail to perform satisfactorily in meeting its NPDES permit effluent limits, the Permittee shall take such immediate corrective action as may be required by this Division, including the construction of additional wastewater treatment and disposal facilities. Upon completion of constructor_ and prior to operation of this permitted facility, a certification must be received from a professional engineer certifying that the permitted facility has been installed in accordance with the approved plans and specifications. Mail the certification to the Permits and Engineering Unit, P.O. Box 27687, Raleigh, N.C. 27611-7687. A copy of the approved plans and specifications shall be maintained on file by the permittee for a minimum of five years from the date of issuance.
