to receiving waters designated as an unnamed tributary to the Deep River in the Cape Fear River Basin
in accordance with effluent limitations, monitoring requirements, and other conditions set forth in Parts
I, II, III and IV hereof.
This permit shall become effective October 12, 2010.
This permit and authorization to discharge shall expire at midnight on August 31, 2011.

SUPPLEMENT TO PERMIT COVER SHEET
All previous NPDES Permits issued to this facility, whether for operation or discharge are hereby
revoked, and as of this issuance, any previously issued permit bearing this number is no longer
effective. Therefore, the exclusive authority to operate and discharge from this facility arises under the
permit conditions, requirements, terms, and provisions included herein.
SIP Hidden Forest, LLC is hereby authorized to:
1. Continue to operate an existing 0.027 MGD wastewater treatment facility with the following

This facility is located near Randleman off the Holder Inman Road Extension at the Hidden Forest
Estates WWTP in Randolph County.
2. Discharge from said treatment works at the location specified on the attached map into an unnamed
tributary to the Deep River, classified C waters in the Cape Fear River Basin.

1. Upstream = at least 100 feet upstream from the outfall. Downstream = at least 100 feet downstream from the outfall.
2. The daily average dissolved oxygen effluent concentration shall not be less than 6.0 mg/L.
3. Limits and monitoring apply only if chlorine is added to the treatment system.
4. Compliance shall be based on a quarterly average of weekly samples.
5. The pH shall not be less than 6.0 standard units nor greater than 9.0 standard units.
THERE SHALL BE NO DISCHARGE OF FLOATING SOLIDS OR VISIBLE FOAM IN OTHER THAN TRACE AMOUNTS.
A. (2.) PERMIT RE -OPENER: SUPPLEMENTARY MONITORING
The Division may, upon written notification to the Permittee, require additional effluent monitoring that it
deems necessary to support its water quality protection and restoration efforts in the receiving stream.

To Whom It May Concern:
This letter is being written as notice that Hidden Forrest Manufactured Home
Community LLC is no longer the owner of the property where the above referenced
permit is held. The property was foreclosed on by the lender in September 2010 and has
been under the management of a receiver since June 21512010. Our company has had no
active management in the W WTP since prior to the assignment of the receiver. At this
time we wish to be taken off of the permit as a responsible party.
Furthermore, we have received the following Notices of Violation and Assessments of
Civil Penalty during this period when we have not supervised the operation of the
WWTP or its operator.
LV-2010-0311 Dated September 13
LV-2010-0280 Dated August 23
LV-2010-0281 Dated August 23
We have not responded to these assessments as we are not in control of the system. We
have forwarded these to the management company currently managing the property.
Please give us direction if we are to do anything further with these items.
