Haywood County
The Division of Water Quality (the Division) hereby issues the attached NPDES permit for the
subject facility. We issue this permit pursuant to the requirements of North Carolina General Statute
143-215.1 and the Memorandum of Agreement between North Carolina and the U.S.
Environmental Protection Agency dated October 15, 2007, or as subsequently amended.
The Division understands that you have made no significant alterations to your wastewater
treatment facilities during the previous permit cycle. Based on your renewal application received
September 7, 2010, we have therefore made only minimal changes to your permit by updating your
facility map and adding parameter codes to the effluent monitoring pages [see Sections, A. (1.) and
A. (2.)]
If any parts, measurement frequencies, or sampling requirements contained in this permit are
unacceptable, you have the right to an adjudicatory hearing, upon written request submitted within
thirty (30) days after receiving this letter. Your request must take the form of a written petition
conforming to Chapter 150B of the North Carolina General Statutes, and you must file it with the
office of Administrative Hearings, 6714 Mail Service Center, Raleigh, North Carolina 27699-6714.
Unless such a demand is made, this permit shall be final and binding.
This permit is not transferable except after notifying the Division of Water Quality. The Division
may require permit modification, or revocation and re -issuance. Please note that this permit does
not affect your legal obligation to obtain other permits required by the Division of Water Quality,
the Division of Land Resources, the Coastal Area Management Act, or other federal or local
governments.

In compliance with the provisions of North Carolina General Statute 143-215.1, other lawful standards
and regulations promulgated and adopted by the North Carolina Environmental Management
Commission, and the Federal Water Pollution Control Act, as amended,

Dogwood Trails Subdivision WWTP
NCSR 1306, N of US Hwy 19, Maggie Valley
Haywood County
to receiving waters designated as Evans Branch in the French Broad River Basin in accordance with
effluent limitations, monitoring requirements, and other conditions set forth in Parts I, II, III and IV
hereof.
This permit shall become effective July 1, 2011.
This permit and authorization to discharge shall expire at midnight on January 31, 2016.

SUPPLEMENT TO PERMIT COVER SHEET
All previous NPDES Permits issued to this facility, whether for operation or discharge are hereby
revoked. As of this permit issuance, any previously issued permit bearing this number is no longer
effective. Therefore, the exclusive authority to operate and discharge from this facility arises under the
permit conditions, requirements, terms, and provisions included herein.

1. continue to operate an existing 0.020 MGD extended -aeration package plant for 100%-
domestic subdivision wastewater treatment consisting of
• -aeration basin
• clarifier
• chlorine -contact chamber with tablet chlorinator
• tablet dechlorinator, and
• digester
located at the Dogwood Trails Subdivision, off NCSR 1306, 0.95 miles north of US Hwy 19 in
Maggie Valley, Haywood County, and
2. discharge from said treatment works at the location specified as Outfall 001 on the attached map,
into Evans Branch [stream segment 5-26-9], a waterbody currently classified C-Trout within
subbasin 04-03-05 of the French Broad River Basin.

• Compliance is good - BIMS DMR Report Violations (2006-2010) indicate four (4) monitoring
frequency violations only; all violations explained by ARO as `BIMS calculation errors" (see
attached compliance summary) - No Notices of Violation / no penalty assessments.
• Effluent data within the bounds of the permit except Total suspended Solids (TSS) show two (2)
accidences of Daily Max limit of 30 mg/L (not assessed) on 29Dec2010 (39.5 mg/L) and 29
Dec2009 (35.5 mg(L), but TSS averages about 12.3 mg/L for the period 2009-2011. Ammonia in
effluent reported max at 17.1 mg/L but averages <1.7 over the last two years. No changes
recommended.

Sludge that is generated during the wastewater treatment process is pumped out of the
clarifier or aeration basin in the wastewater treatment plant. When the sludge has to be
removed a septage management firm is called in to pump the digestor and dispose of the
solids at the County Municipal Treatment Plant.
