Mason Subject:RE: BR-0160 Utility Bore Frac Out 20230516 (1 of 2) Date:Wednesday, May 17, 2023 2:37:50 PM Hannah, Glad that the exhibits are beneficial. We will send out pictures of the final clean up. Lee construction has completed their bore work for this site, so to answer your question they likely do not have a plan for moving forward on this project. However, there work has identified a rock layer that will be good information to provide prior to the wet utility bore contractors. Sincerely, Jon Jon W. Once clean-up has been completed, please update us with additional pictures to add to the project file. Does the contractor have a plan in place if additional frac outs occur (similar to the one at NC 211)? The frac out occurred near STA. 18+00 RT, which resulted in a discharge of approximately 50 gallons of bentonite mix, averaged dimensions are 4’W x 60’L x 3”D. The frac out occurred in an old ditch in the woods located on the easement/ROW line it wasn’t until the material exited the wood line that the crew identified the issue. As soon as the frac out was identified, the crew shutdown operations and began immediate containment. They contacted Tim Godwin with NCDOT. Tim remained onsite until the material was contained and clean up efforts were well under way. The bore operation was permitted to resume post containment and initial clean up. The contractor did add additional vac trucks to the project before continuing the operation to manage any additional material as needed. The crew worked through the night to complete pulling product and continue their clean up efforts. I have included a few images of the frac out and initial cleanup efforts. It appears most of the bentonite mix was within permitted impact areas. I will send additional photos of post clean up conditions as soon as I get them, likely tomorrow (05/18/2023). If you have any additional questions or need any additional information please reach out. Sincerely, Jon Jon W.
