This letter is to inform you that the Division has finalized its review and the comment period is
now closed for NPDES permit NC0038709. A final copy of the permit is attached to this letter. This
permit is issued pursuant to the requirements of the North Carolina General Statute 143-215.1 and the
Memorandum of Agreement between North Carolina and the US Environmental Protection Agency
dated December 6, 1983.
The following list has been included to outline the changes that were addressed in the draft
permit. These changes remain unchanged in the final permit. In addition, the revisions outlined in the
comment letter received on May 7, 1999 have also been addressed.
➢ The permit expiration date has been changed. The permit renewal will expire on September
30, 2003. The expiration date has been revised so that an equal number of permit renewals
will come in at any one time. This change will allow the Division to issue permits more
efficiently.
➢ The stream classification for the Yadkin River, at the location of your point source discharge,
has been changed from WS-IV to WS-V. The reclassification became effective August 1,
1998.
➢ The monitoring frequency for pH has been changed from weekly to 2/month. This change is
consistent with the requirements for Class I facilities as outlined in the North Carolina
Administrative Code 2B.0500.
➢ Quarterly monitoring for Total Nitrogen and Total Phosphorus has been included in the draft
permit. The Division has included these monitoring requirements for all dischargers in the

Yadkin Basin. The results of this additional monitoring will be used in the development of a
nutrient management strategy for the Yadkin Basin.
If any parts, measurement frequencies or sampling requirements contained in this permit are
unacceptable to you, you have the right to an adjudicatory hearing upon written request within thirty (30)
days following receipt of this letter. This request must be in the form of a written petition, conforming to
Chapter 150B of the North Carolina General Statutes, and filed with the Office of Administrative
Hearings, Post Office Drawer 27447, Raleigh, North Carolina 27611-7447. Unless such demand is
made, this decision shall be final and binding.
Please note that this permit is not transferable except after notice to the Division. The Division
may require modification or revocation and reissuance of the permit. In addition, this permit does not
affect the legal requirement to obtain other permits which may be required by the Division of Water
Quality or permits required by the Division of Land Resources, Coastal Area Management Act or any
other Federal or Local governmental permit that may be required.

RESPONSE:
l/ This agency has reviewed the draft permit and determined that the proposed
discharge will not be sufficiently close to any existing or know proposed
public water supply intake so as to create an adverse effect on water quality.
We concur with the issuance of this permit provided the facility is operated
and maintained properly, the stated effluent limits are met prior to discharge,
and the discharge does not contravene the designated water quality standards.

Per our conversation on 05!07/99 regarding "Supplement To Permit Cover Shcct" please
delete the section of our permit that indicated "4,200 GPD dual bed recirculating sand

compliance with some minor modifications by our ORC and we decided to forego the
$20,000.00 recirculation system at Roaring River School, presently.

#3, Advance, NC, Davie County. The facility discharges 0.193 MGD of
treated domestic wastewater from one cuffall into the Yadkin River, a
Class WS-IV CA stream in the Yadkin -Pee Dee River Basin which has a
7010 flow of 546 cfs. No parameters are water quality limited, but this
discharge may affect future allocations.
2. NPDES No. NC0038709. Wilkes County Board of Education, 201 West
Main Street, Wilkesboro, NC 28697 has applied for a permit renewal for a
facility located at the Roaring River Elementary School, 283 White Plains
Road, Roaring River, Wilkes County. The facility discharges 0.0045 MGD
of treated domestic wastewater from one ouffall into the Yadkin River, a
Class WS-V stream in the Yadkin -Pee Dee River Basin. No parameters
are water quality limited, but this discharge may affect future alloca-
tions.
3. NPDES NO. NC0065587. Mid South Water Systems, Inc., 3502 Mt.
Pleasant Road, Sherrills Ford, NC 28673 has applied for a permit renew-
al for a facility located at Frye Bridge Wastewater Treatment Plant on
NCSR 2998, Clemmons, Forsyth County. The facility discharges 0.027
MGD of treated domestic wastewater from one ouffall into an unnamed
tributary to Muddy Creek, a Class WS-IV stream in the Yadkin -Pee Dee
River Basin. For some parameters, the available load capacity of the im-
mediate receiving water will be consumed. This may affect future water
quality based effluent limitations for additional dischargers within this
portion of the watershed.
4. NPDES No. NC0033162. Davie County Board of Education, 220 Cher-

ry Street, Mocksville, NC 27028 has applied for a permit renewal for a fa-
cility located at Shady Grove Elementary School at 3179 Cornaher
Road, Advance, Davie County. The facility discharges 0.0115 MOD of
treated domestic wastewater from one outfall into an unnamed tributary
to Carter Creek, a Class C stream in the Yadkin -Pee Dee River Basin
which has a 7010 flow of 0 cis and 3002 flow of 0 cis. BOD5, ammonia,
and dissolved oxygen are water quality limited. For some parameters,
the available load rapacity of the immediate receiving water will be con-
sumed and may affect future water quality based effluent limitation

Summary
Facility has submitted an application for renewal with no identified changes to facility

• 303(d) List Review: Facility is not listed on North Carolina's 1998 303(d)
List.
• Review of Basin Plan:
The Yadkin River and many of its tributaries in the vicinity of Ronda and
Elkin is support threatened due to fecal coliform levels well over the
standard.
The recommendation given in the Plan really only applies to the Wilkesboro
and the North Wilkesboro facilities (they have been in violation of their fecal
coliform limits for several years). The recommendation was to combine
these two discharges into one outfall into the Yadkin River. In addition, the
plan states that additional data must be collected for settleable solids and
instream biota at ABT Co. to determine if the river has recovered below the
discharge.
Staff Report (most recent was 1993): Facility is classified as Class I. Staff
report indicated that the discharge has been relocated to the Yadkin River.
The discharge point is approximately one mile downstream from Abitibi
Price's discharge point.
• WLA Review (Completed 3-11-91):
Limits were detemined to be secondary. No instream monitoring was
recommended. The discharge was relocated from an UT to Yadkin River to
the Yadkin River. 7Q10 is 276 cfs.
• DMR Review:
Two BOD violations reported in December 1998. Flow levels appear to be
within the constraints of the existing permit limit.
BOD levels appeared to be somewhat high in 96 and 97. BOD levels range
from 71 mg/l to 9 mg/l in 96 and 97. Flows also appear to be high. In 1997
several months had average flows at or near the permitted level of 0.0045
MGD. Based on the 98 DMR data, it appears that these problems have been
corrected.

Proposed upgrade includes a recirculation tanks) and a pump station with controls. A
flow meter may be installed to measure the flow of the effluent discharge.
3. Residuals handling and utilization/disposal scheme: The school uses their own pump
truck to pump the septic tank at the beginning of each summer. The residuals are then
hauled to the North Wilkesboro WWTP.

June 19, 1991. It allowed for the construction of a 20
gpm effluent pump station with dual submersible pumps,
a high water alarm, and stand by power (portable
generator). It also allowed for the construction of
3,510 feet of two inch force main, 18 feet of four inch
force main, rip -rap scour pad, and all the associated
piping, valves, and appurtenances. The construction
was finished on December 14, 1991.

PART IV' - EVALUATION AND RECOMMENDATIONS
This school has approximately 407 staff and students. Since
the last staff report the discharge has been relocated to the
Yadkin River. The discharge point is approximately one mile
downstream from Abitibi Price's discharge point. On the day of
the inspection the system was well maintained. There was no
visible environmental impact at the discharge point.
The regional office has no objection to this permit being
reissued.

Dear Mr. Weaver:
With this letter the Wilkes County Board of Education is requesting renewal of the above
referenced NPDES permit.
Enclosed is the renewal applications, delegation of authority letter, sludge management
control plan letter and check # 041196 in the amount of $2,000.00. This fee amount is
for the purpose of renewal for all 10 of our NPDES permits at $200.00 each of which the
other 9 applications are part of this complete renewal package.
In item (3) of the permit applications for NPDES permits NC0046388, NCO046426 and
NC 0033138 I have also checked areas for expansion/modification. The reason for this is
when those facilities were upgraded, chlorination was replaced with ultraviolet disinfection.
Possibly due to an administrative oversight the weekly and bi-weekly chlorine residual
samplings were not taken from the permits at that time. We request these chlorine residual
samplings be removed from the new permit.
Other than the information I have provided there has been no other expansions /
modification to other facilities. Please contact me if you have further questions.
