Accordingly, we are forwarding the attaehed NPDES discharge permit. This permit is issued pursuant to the requirements of Ncrth Carolina General Statute 143-215.1 and the Memorandum of Agreement between North Carolina and the U.S. Environmental Protection Agency dated October 15, 2007 (or as subsequently amended). This final permit includes no changes from the draft permit sent to you on July 7, 2010. If any parts, measurement frequencies or sampling requirements contained in this permit are unacceptable to you, you have the right to an adjudicatory hearing upon written request within thirty (30) days following receipt of this letter. This request must be in the form of a written petition, conforming to Chapter 150B of the North Carolina General Statutes, and filed with the Office of Administrative Hearings (6714 Mail Service Center, Raleigh, North Carolina 27699-6714). Unless such demand is made, this decision shall be final and binding. Please note that this permit is not transferable except after notice to the Division. The Division may require modification or revocation and reissuance of the permit. This permit does not affect the legal requirements to obtain other permits which may be required by the Division of Water Quality or permits required by the Division of Land Resources, the Coastal Area Management Act or any other Federal or Local governmental permit that may be required. Everything looked good. Attached is that inspection report. The 2009 SPAR (System Performance Annual Report) was received by the Division on June 21, 2010. I feel that everything else in the attached documents is accurate. ARO recommends renewal of this permit. Is this email sufficient? SUJBECT: NPDES NCO039152 Application Cover Letter DATE: March 10, 2010 Ohio Electric Motors, Inc. is officially requesting renewal of NPDES Permit # NCO039152 to continue wastewater discharge into Paint Fork Creek. We currently have 60 employees and our waste water output consist of only domestic waste from the washrooms and sinks for outfall 001. This system consist of a Septic Tank, Sand Filter, Tablet Chlorinator, Tablet De -chlorination system, backflow check valve to prevent high water conditions from flowing from the creek back into the systems and effluent discharge pipe. Outfall 002 is a discharge of clean water used for machine cooling and is monitored for pH and temperature only per permit requirements. Our system has been improved since the last permit application with the addition of a 1) de -chlorination system to neutralize any chlorine discharge into Paint Fork Creek and 2) a backflow check valve to prevent any flood waters from flowing back through the effluent pipe into the system. No other modifications have been made and monitoring indicates the system is working as designed well within the permit discharge limits. All required items for the renewal package have been completed accurately to the best of my ability.
