Good Afternoon Bethany,
This has been re -signed. Please see attached. Hardcopy is in the mail.
Our client also found what appears to be the original hardcopy in his records, that he signed in October last year, and
believes it was mailed in ... So I'm wondering if the wrong copy (one with a digital signature) got uploaded, while you
might have the hardcopy with a wet signature? I've attached the one from October too, just to make sure everything
matches up appropriately if needed.
Hopefully I didn't make that more confusing!
Thanks!

Good Morning Bethany,
I just got this notification — this occurred prior to my starting on this project... Should I go ahead and both email and mail
the hardcopy of this form with the original signature?
Thanks!
