On May 20, 2015, the Division of Water Resources (Division) received your application dated
March 30, 2015 requesting a 401 Water Quality Certification from the Division for your project.
The Division has determined that your application is incomplete and cannot be processed. The
application is on -hold until the following information is received:
1. The site plan must show the entire project not just the jurisdictional stream impact in
order to justify the road crossing for access to infrastructure. [15A NCAC 02H .0502(g)]
2. Exhibit 1 shows the culvert placed at the existing streambed elevation. Per the
conditions of Water Quality Certification #3886 (GC 3886), culverts must be below the
elevation of the streambed by one foot for culverts with a diameter greater than 48
inches. [15A NCAC 02H .0502(g)]
3. Per the requirements of GC 3886 for low density projects, the project shall meet the
appropriate criteria described at the beginning of the low density form (link below).
Please provide the grading plan for this project so that how the stormwater is being
conveyed can be evaluated. [15A NCAC 02H .0506(b)(5) and (c)(5)]

4. Per the requirements of Attachment Ain GC 3886, please submit two copies of the DWR
Low Density Supplement Form with all required items found at
http: / /Portal.ncdenr.org /web /Ir /state - stormwater -forms docs. [15A NCAC 02H
.0506(b)(5) and (c)(5)]
Pursuant to Title 15A NCAC 02H .0502(c) and Title 15A NCAC 0213 .0233 (8), the applicant shall
furnish all of the above requested information for the proper consideration of the application.
If all of the requested information is not received in .writing within 30 calendar days of receipt
of this letter, the Division will be unable to approve the application and it will be returned. The
return of this project will necessitate reapplication to the Division for approval, including a
complete application package and the appropriate fee.
Please respond in writing within 30 calendar days of receipt of this letter by sending four copies
of all of the above requested information to the 401 & Buffer Permitting Unit, 1617 Mail Service
Center, Raleigh, NC 27699 -1617.
Please be aware that you have no authorization under the Water Quality Certification Rules or
the Neuse Riparian Buffer Rules for this activity.
