Sullins, Director Division of Water Quality October 26, 2007 DWQ Project # 07-1557 Haywood County CERTIFIED MAIL: RETURN RECEIPT REQUESTED Mr. Nick Roknich Blue Sky Communities, Inc. 89 Walnut Street Waynesville, NC 28786 Subject Property: Rock Bridge -AFTER-THE-FACT Ut to Hemphill Creek [040305, 5-26-16, C, TR] REQUEST FOR MORE INFORMATION Dear Mr. Roknich: On October 17, 2007, the Division of Water Quality (DWQ) received your After-the-Fact application dated October 8, 2007, to fill or otherwise impact 2401inear feet of perennial stream, to construct the proposed residential development. The DWQ has determined that your application was incomplete and/or provided inaccurate information as discussed below. The DWQ will require additional information in order to process your application to impact protected wetlands and/or streams on the subject property. Therefore, unless we receive five copies of the additional information requested below, we will have to move toward denial of your application as required by 15A NCAC 2H .0506 and will place this project on hold as incomplete until we receive this additional information. Please provide the following information so that we may continue to review your project. Additional Information Requested: Please re-submit your site plans on full plan sheets at a scale of no smaller than 1 "=50'. 2. Please provide cross section details showing the provisions for aquatic life passage. 3. Page 2 of 2 October 26, 2007 5. Please indicate all stream impacts including all fill slopes, dissipaters, and bank stabilization on the site plan. 6. Please indicate all wetland impacts including fill slopes on the site plan. ~.;: 7. Your proposed stream crossings Nos. 1, 2, 3 and 4, appear to be aligned at outside of the 75° and 105°. Please re-configure your proposed stream crossing to show it aligned approximately perpendicular (between 75° and 105°). 8. Please submit impacts for waterline installation. 9. Please provide the non-web based Haywood County Soil Survey map for the site. 10. Please verify that the USACE will permit the stream restoration under a NW29 Please submit this information within 30 calendar days of the date of this letter. If we do not receive this requested information within 30 calendar days of the date of this letter, your project will be withdrawn and you will need to reapply with a new application and a new fee. This letter only addresses the application review and does not authorize any impacts to wetlands, waters or protected buffers. Please be aware that any impacts requested within your application are not authorized (at this time) by the DWQ.
