Facility Summary Hebron Colony runs a Rehab Facility for about 30 residents with a private Hebron Colony & Grace Home WWTP on site. The Hebron Colony WWTP is a minor facility (flow < 1 MGD) with a permitted wastewater discharge of 0.009285 MGD. The WWTP was originally issued in 1981. Compliance History Hebron Colony & Grace Home WWTP has only had 1 NOV in past 5 years, and it was in 2018. DO was low in 2 aeration tanks - about 0.8. Comments on Draft Permit (Peer Review notes from Sydney Carpenter) Box • Make sure to be including the Stream Index number and HUC code on the Supplement to Cover Sheet. The Stream Index can be found on the Outfall window in BIMS. The HUC can be found using the 'Find Your HUC' GIS site or the 'DWR Map' GIS site. I've added it to draft permit for you so you can see where I normally put it. This makes it a huge help in identifying the exact stream for later renewals or any questions we might get! • The facility's Grade/Classification was missing under A. (1.). Older permits won't have this, so it's something to look out for and add when you're doing a renewal. You can find this info on the 'Class/Desig.' tab in BIMS. WW-2 means it's a Grade II Biological facility. If you ever see a facility classification as'PC-1', that means it's a Physical/Chemical classified facility (15A NCAC 08G .0306). • We usually always format the Effluent table using the'Arial Narrow' font to help everything fit. The rest of the permit stays in Times New Roman. • One thing I forgot to mention regarding the effluent table is that we've been updating where it says 'Parameter' to 'eDMR' for the code now that permittees can submit through our eDMR system. Factsheet & Cover letter • See the comments I've added to the documents! All minor corrections. Map The receiving stream name and receiving stream index # are missing from the map. I'd update it to include this information for easy reference (Notes from Regional Office) From Jenny Graznak — last inspection was 2019 and was unremarkable. Site is not a repeat violator.
