Road: NC 56 just west of South Granville High School County: Granville River Basin: Neuse Stream Name: Whitaker Branch oo Lat, Long: 36.12197N, 78.67723W HUC: 03020201 NCDENR-DWQ Index Number, Stream Classification: 27-12-1-2, WS-IV Existing conditions: fill slope failure adjacent to stream Proposed repair: Rebuild the shoulder and fill slope by plating the slope with rip rap. Note: Rip rap will be installed underlined with filter fabric for long term stability. Additionally rip rap will be necessary to restore the stream bed in the plunge pool and provide stability for the wing walls so that they aren’t further undermined. No geotextile will be placed across stream bed. Dewatering: Sandbags will be installed upstream and downstream of the work site as needed for containment. Due to low/no flow conditions a pump around operation will not be necessary. Note: existing stream conditions are much drier than they were back in March when the attached photograph was taken. Impervious dike materials used for containment will be in place for as long as necessary, but will be removed prior to project completion. 1 Permanent impacts: 20ft of surface water fill in plunge pool to restore stream bed No buffer impacts are proposed (work will be performed within existing transportation facility and within an existing highly maintained utility corridor) 20ft of (rip rap banks only to correspond with plunge pool rip rap) Temporary impacts: 20ft dewatering Protected species information: Biological Conclusions of No Effect have been rendered for the Dwarf wedge-mussel, smooth coneflower, and Harperella based on lack of habitat for protected species at this location. Miscellaneous notes: ◦ Contact Division Office 5 environmental staff immediately should conflicts between this proposal and the permit conditions arise. For maintenance sites such as this one, NCDOT Division 5 has a procedure in place to provide our construction crews with print outs of the general conditions along with a letter head cover sheet explaining the intent to operate under a non-reporting permit as long as all applicable conditions are met. Oversight from both the Environmental Staff and REU Field Operations Unit will ensure compliance with the permit conditions are met and that standard BMPs are followed. nd This work is scheduled to begin Monday June 2.
