Per 15A NCAC 02T.0105(c)(6), directly related environmental permits or certification applications must be being prepared,

Per 15A NCAC 02T.0402, "high -priority sewer" means any aerial sewer, sewer contacting surface waters,
siphon, or sewers positioned parallel to streambanks that are subject to erosion that undermines or deteriorates the sewer.
Siphons and sewers suspended through interference/conflict boxes require a variance approval.
If yes, include an attachment with details for each line, including type (aerial line, size, material, and location).
High priority lines shall be inspected by the permittee or its representative at least once every six -months and
inspections documented per 15A NCAC 02T.0403(a)(5) or the permittee's individual System -Wide Collection permit.

the volume of wastewaterto be permitted in this project has been evaluated along the route to the receiving
wastewater treatment facility and that the flow from this project is not anticipated to cause any capacity
related sanitary sewer overflows or overburden any downstream pump station en route to the receiving
treatment plant under normal circumstances, given the implementation of the planned improvements
identified in the planning assessment where applicable. This analysis has been performed in accordance
with local established policies and procedures using the best available data. This certification applies to
those items listed above in Sections I and II phis all attached planning assessment addendums for which I

SUBJECT: FAST TRACK GRAVITY SEWER CONSTRUCTION PERMIT
PARK RIDGE (NC HWY 268W- Surry County)
ADI PROJECT: 22-034
On behalf of the Yadkin Valley Sewer Authority, we are pleased to submit the enclosed information for
permitting of the private sanitary sewer line extensions to serve the subject project.
The proposed sewer extension will serve 45 residential single-family homes. The lines will be
constructed with 8" PVC and 8" DI pipe at the right of way crossing. The lines will be publicly
maintained by the Yadkin Valley Sewer Authority. The required flow allocation is based on 360
gallons per day for the 45 (forty-five) three -bedroom homes for a total flow of 16,200 gallons per day.
Please find enclosed:
1. This cover letter with the project narrative description in the paragraph above;
2. A check in the amount of $480.00 made payable to the NCDEQ as Payment of the
application review fee, for the PUBLIC portion of the sewer extension;
3. One original and one copy of a completed and executed "Fast Track Sewer System
Extension Application FTA 06-21 ", for gravity sewers, pump stations and force mains,
for the private sewer extension;
4. One original and one copy of a completed "Flow Tracking for Sewer Extension
Applications" Form FTSE — 10-18, for the private sewer extension;
5. One color copy of the USGS topographic map of this area;
6. A street level map showing the general project area;
7. And, a CD containing a pdf of the application and all supporting documentation.
