The City has recognized that the stream channels at these sites are experiencing severe bank erosion, which is jeopardizing City -owned underground utilities and infrastructure. The City proposes to implement stabilization measures that would protect sanitary sewer infrastructure and other utilities at the sites, improve aquatic habitats at the sites, and improve water quality downstream. The three projects include: Site 1, Mill Creek at Reynolda Road; Site 2, Salem Creek at Weatherwood Lane; and Site 3, Unnamed tributary to Salem Creek at the Archie Elledge Wastewater Treatment Facility. The proposed streambank stabilization work will utilize natural channel design techniques, and will include boulder cross vanes, boulder single arm cross vanes, floodplain benching, bank sloping, vegetated geolifts, brush mattresses, live stake planting, and minor channel realignment. The City proposes to execute these projects under Nationwide Permit 27 (Aquatic Habitat Restoration, Establishment, and Enhancement Activities). Describe the existing conditions on the site and the general land use in the vicinity of the project at the time of this application: The Mill Creek Project Site (Site 1) is located immediately downstream stream of the Reynolda Road Bridge (Sheet MC 2.0). The upland on stream right is a developed shopping area. On stream left, the area between the streambank and Reynolda Road is vegetated with mature forest canopy and a greenway. The stream is crossed by Reynolda Road bridge at the upstream end of the work area. The drainage area measures 26.2 square miles with approximately 19.1 % impervious surface cover. Extreme erosion is occurring along the right streambank adjacent to the development for approximately 130 feet. The bank ranges between 10 and 12 feet in height (from water surface) and is encroaching on a 30 inch sewer line and a driveway serving the development. Less extreme streambank erosion is present on left and right banks further downstream. Several utilities exist on the Mill Creek Project Site, the most prominent of which are sewer and gas. 3.1 b. List the total estimated acreage of all existing wetlands on the property: 0 acres 3.1 c. List the total estimated linear feet of all existing streams (intermittent and perennial) on the property: 970 feet 3.1 d. Explain the purpose of the proposed project: This project is proposed to address severe bank erosion and mass wasting on the right bank of Mill Creek that is threatening subsurface utilities, developed areas, and contributing substantial sediment loads to the watershed. Through the use of natural channel concepts, the streambanks will be stabilized to halt the erosion. The project will also include steps to strengthen and upgrade the existing sewer pipes located on stream left and crossing the stream at the bridge. 3.1 e. Describe the overall project in detail, including the type of equipment to be used: Streambank stabilization at Mill Creek: i. Prior to the start of soil or sediment disturbing work, rock check dams will be installed to span the channel downstream of the work area to create sediment traps (Detail 3/8.0) and erosion control measures will be installed (Sheet 8.1). These will be temporary structures, and are to be removed after the project is complete. ii. Clearing and debris removal will be conducted as needed to execute the goals of the project, however great care will be exercised not to disturb the stream or to unnecessarily damage existing vegetation. All debris generated through this work will be removed from the site. iii. The right streambank slope will be cleared of vegetation to allow access to the work area. The angle of the slope will be reduced to decrease the erodability. All exposed soil will be planted with an herbaceous seed mix to stabilize the soil after the work is complete (Sheet MC 5.0). iv. Two rock vanes will be installed spanning from the thalweg to tie into the left bank above % bankful stage height in the curve (Sheet MC 4.0 and Detail 3/10.0). The rock vanes will be installed at an ascending angle from stream bed to the bank at an 8% slope, at a 20 -30 degree angle to the thalweg, such that the energy of high flow events will be turned away from the bank toward the stream center and dissipated, reducing the erosive force exerted against the bank. Rock structures will be installed using track hoes and hand labor. The toe of slope space behind the rock vanes will be filled in with vegetated geolifts (Detail 4/10.1), and a brush mattress structure will be installed at the downstream end of the downstream rock vane to provide additional bank protection at the tail end of the curve (Detail 5/10.1). The area around the installed structure will be planted with bare root and live stake plantings of native woody species (Sheet MC 5.0). Rock structures and plantings will be installed using track hoes and hand labor. V. The 36 inch RCP sewer line on stream right will be rehabilitated with a cured in place lining inside the existing pipe. This work will not cause temporary or permanent impacts the stream (Sheets MC 4.0 and SC 7.1). vi. The sewer line that currently crosses Mill Creek within the bed on the downstream side of the Reynolda Road Bridge will be improved by installing a restrained joint ductile iron pipe inside the existing 24 inch RCP and filling the annular space with a grout material. This work will be accomplished from the manhole locations on the upland areas on both sides of the stream and will not cause temporary or permanent impacts the stream (Sheets MC 4.0 and SC 7.1). 1.2 Property Identification: Site 2, Salem Creek at Weatherwood Lane 1.2a. Describe the existing conditions on the site and the general land use in the vicinity of the project at the time of this application: The Salem Creek project site (Site 2) is located immediately upstream of the 1 -40 bridge over Salem Creek (Sheet SC 4.0). Land cover on both sides of the stream in the work area is forested, however upstream of the project reach is the urban area of the City of Winston - Salem. The drainage area measures 54.8 square miles with approximately 23.2% impervious surface cover. Extreme erosion is occurring along the right streambank for approximately 200 feet. The bank ranges between 6 and 8 feet in height (from water surface) and is threating the integrity of a 36 inch raw water line, which Winston -Salem Utilities intends to rehabilitate. The primary access road into Site 2 is from the end of Weatherwood Lane to the south east of the site. Weatherwood Lane terminates into a dirt road which runs through an existing Duke Energy right of way (ROW) (Sheet SC 4.0). This access road is ordinarily used to service the high tension power lines in the ROW and traverses a forested wetland area that drains to Salem Creek. The secondary access to Site 2 is from a dirt road off the end of Fairfield Road to the north east of and on the opposite side of I -40 from the site. This dirt road is on an existing City of Winston Salem sewer ROW and goes under the 1 -40 bridge on stream left. 3.2b. List the total estimated acreage of all existing wetlands on the property: 0.5 acres 3.2c. List the total estimated linear feet of all existing streams (intermittent and perennial) on the property: 760 feet 3.2d. Explain the purpose of the proposed project: This project is proposed to address severe bank erosion and mass wasting on the right bank of Salem Creek that is threatening subsurface utilities and contributing substantial sediment loads to the watershed. Describe the existing conditions on the site and the general land use in the vicinity of the project at the time of this application: The Unnamed tributary to Salem Creek (UT -SC; Site 3) project site is located on the Archie Elledge Wastewater Treatment Plant (WWTP) (Sheet UT 2.0). The drainage area measures 1.13 square miles with approximately 15.5% impervious surface cover. The stream is experiencing extreme erosion on both banks for approximately 1,380 feet. Several fence posts have been undermined and are compromising the integrity of and existing chain link fence. Additional WWTP infrastructure could be affected if this site is not addressed. Land cover on the slope adjacent to stream left is forested with mature canopy, although the streambanks are primarily vegetated with shrubs, vines, and invasive woody species. The WWTP is on stream right and the land cover is primarily developed and impervious surface consisting of paved road surfaces, parking areas, buildings, and other WWTP infrastructure. 3.3b. List the total estimated acreage of all existing wetlands on the property: 0 acres 3.3c. List the total estimated linear feet of all existing streams (intermittent and perennial) on the property: 1380 feet 3.3d. Explain the purpose of the proposed project: This project is proposed to address extreme bank erosion and mass wasting on both banks of UT- SC that is threatening infrastructure at the WWTP, and contributing substantial sediment loads to the watershed. The perimeter fence has been compromised and a utility pole is in the process of becoming undermined. The paved WWTP road adjacent to the stream is at risk, as well as critical subsurface pipes and other WWTP components. Through the use of natural channel concepts, the streambanks will be stabilized to halt the erosion. 3.3e. Describe the overall project in detail, including the type of equipment to be used: Streambank stabilization at UT -SC: i. Prior to the start of soil disturbing work, erosion control measures will be installed (Sheet 8.1). These will be temporary structures, and are to be removed after the project is complete. ii. Clearing and debris removal will be conducted as needed to execute the goals of the project, however great care will be exercised not to disturb the stream or to unnecessarily damage existing vegetation. All debris generated through this work will be removed from the site. iii. All work in- channel work will be done in dry conditions using a pump around and special stilling basin operation for flow diversion (Detail 2/8.0). iv. Design details include installation of cross vane structures at the upstream end of project reach, and 5 additional locations (Detail 2/10.0) to concentrate flow to the center of the channel and reduce erosive velocities. Near the upstream end of the project, concentrated stormwater flow from the developed area enters the stream channel on the right bank. A boulder step structure will be installed to safely convey this flow to the project channel (Detail 2/8.0). Eight rock vanes will be installed in critical curves (Detail 3/10.0), four of which will have additional bank stabilization on stream right with vegetated geolifts (Detail 3/10.1). Two constructed riffle sections will be installed to establish and maintain grade control (Detail 4/10.0). The entire right bank of the project will be graded to establish a stable slope and planted with live stakes and seed mix to stabilize (Sheets UT 4.0 and 4.1). Avoidance and Minimization 1 a. Specifically describe measures taken to avoid or minimize the proposed impacts in designing project. All proposed impacts to jurisdictional stream channels are related to stabilizing eroding streambanks and restoring habitat at the three project sites. Multiple engineering alternatives were considered, including hardened bank structures, but natural channel design parameters were selected to most effectively accomplish project goals. The project proposes temporary impacts to one jurisdictional wetland related to construction of the primary access road at Site 2. Other options were investigated, however, due to the site location, no other route to gain access to the project site on the right bank of Salem Creek is feasible. The secondary access route planned for the left bank of the project site does not impact jurisdictional waters, but will not give access to the right bank without construction of a large ford structure in the channel and creation of a ramp down the left bank, thereby incurring more impacts to the stream. 1 b. Specifically describe measures taken to avoid or minimize the proposed impacts through construction techniques. At Sites 1 and 2, various construction methods for minimizing off - project sediment release during construction of the in- stream structures were considered, including pump around operations and diversion, however these were deemed infeasible due to the flow volume of the stream. Erosion and sediment control during construction will utilize a series of in- stream rock check dams at the downstream end of the work area. Prior to the start of soil or sediment disturbing work, these will be installed spanning the channel to create sediment traps (Detail 3/8.0). These will be temporary structures, and are to be removed after the project is complete. During the course of construction, these will be regularly cleaned out and maintained. The proposed impacts to the jurisdictional wetland area at Site 2 will be minimized by use of mud mats over geotextile filter fabric to create a temporary roadbed. This will be removed after construction is complete and the area will be seeded to return it to pre- construction conditions. 2.
