Dear Mr. Kegley:
Hart & Hickman, PC (H&H) is providing the attached updated monitoring well permit application
for groundwater assessment activities related to the former Heritage Cleaners site in Wilmington,
North Carolina. H&H is proposing to install two (2) permanent monitoring wells, one (1) shallow
(MW-4S) and one (1) intermediate (MW-4D), at 622 South 2nd Street, if final approval by the
owners is obtained. If not obtained, the wells are proposed to be located in the City of Wilmington
Right -of -Way near 615 South 2nd St. The two (2) permanent wells will be constructed next to one
another as a "nested" pair, but will be installed in separate boreholes and protected with separate
water tight caps, flush -mount steel manhole covers, and concrete pads. These two (2) proposed
wells are part of a total of five (5) "nested" pairs of wells for a total of ten (10) permanent wells to
be installed as part of the assessment activities. Enclosed please find the permit application, well
construction details, site map with the proposed location, and other relevant information for the
two proposed wells. Please review the provided information and return the approved permit to the
H&H Raleigh office.

conducted by the Division or its contractors. All monitoring wells will be properly
abandoned in accordance with applicable laws and regulations, unless other
arrangements are agreed to by the Property Owner.
(3)The Division or its contractors will make reasonable attempts to notify the Property
Owner at least 48 hours prior to entering the Property for any purpose. In situations
that the Division determines to be of an emergency nature, the Division or its
contractors shall have immediate access to the property.
(4) Property Owner shall not willingly destroy, damage, remove, pave over or cover any
monitoring wells at the site without prior consent of the Division.
By signing this consent document, I acknowledge that I am the legal property owner
or tenant in residence that has the authority to allow this work on the property and
have contacted all tenants (if there are tenants) occupying the property and all tenants
agree to the conditions of this `Property Access Consent".

Hi Mr. Hammond,
As discussed last week, we are anticipating having to add an additional permanent monitoring well pair (total of two
wells) within the City of Wilmington ROW. These wells may be located on private property, but we would like to submit
an application for approval in the event that they cannot be placed on private property. The permanent wells would be
the same construction as the other wells that we have received approval for. We are hoping to begin installation of
these wells on Monday, May 15th, at the same time as the other approved locations (assuming that the wells cannot be
located on private property). Attached, please find an application, a letter and attachments showing the proposed well
pair location and well construction, and a Google street view of the proposed well pair location. The proposed location
for this well pair would be in the ROW near 615 S 2"d St.
Additionally, I wanted to let you know that after conducting a private utility locate, the well pair that was proposed to be
located near 700 S 3rd Street (Permit #20230305) will need to be moved to the proposed alternate location near 213
Queen Street.

Dear Mr. Hammond:
Hart & Hickman, PC (H&H), under contract to the North Carolina Dry -Cleaning Solvent Cleanup
Act (DSCA) Program, is requesting authorization to install a permanent monitoring well pair in the
City of Wilmington Right -of -Way (ROW) as part of environmental assessment activities associated
with the former Heritage Cleaners located at 522 S. Third Street in Wilmington, North Carolina.
These monitoring wells are in addition to the three pairs of permanent monitoring wells and three
temporary soil gas points that have already been approved by the City, of Wilmington. Up to one
additional permanent monitoring well pair are proposed, including one shallow and one intermediate
well in close proximity, for a total of two permanent monitoring wells. A figure showing the
proposed sampling location is included in Attachment 1.
The work will be conducted in the grassy area of the Right -of -Way (ROW). The permanent
monitoring well borings will consist of 3-inch diameter borings advanced between 15 and 30 feet
below ground surface (ft bgs) using an air knifing truck (parked alongside the road in a street parking
space) and/or small track -mounted drilling rig. Monitoring wells will be set in each boring using f-
inch diameter PVC casing and screen with a sand filter pack and bentonite/grout seal. The
monitoring wells will be protected with flush -mount steel manhole covers and 2-ft by 2-ft concrete
pads. The monitoring wells will remain in -place for long-term groundwater monitoring by the DSCA
Program. Utilities will be located prior to boring activities via NC One Call and a private utility
locator. Proposed monitoring well construction details are included in Attachment 2.
