Dear Mr. Swaim:
On 4/27/2023, a Pretreatment Audit was performed by Tricia Lowery of the Winston-Salem Regional
Office. Grant Trivette, Pretreatment Coordinator, assisted Ms. Lowery and was present during the audit.
The purpose of this audit was to determine the effectiveness of the Control Authority's pretreatment
program regarding SIU inspections., which includes the initial interview, tour of SIU facility, sampling
review, and an exit interview.
Background
The Control Authority has 4 Significant Industrial Users (SIUs), one of which is a 433.15 categorical user
(CIU).
Control Authority Interview
No problems reported that are related to industrial discharges and no industries in SNC over the past 2
semi-annual periods. Three dental practitioners discharge to POTW and have complete compliance
forms. Appropriate amalgam separators are in use. Yadkin County discharges into Town of
Yadkinville's collection system/POTW. No issues from Yadkin County. The POTW receives waste
from groundwater remediation project — Lydall. They have a general local permit with a limit of 10,000
gpd. No issues with Lydall have been reported. All files are in good order and well organized.
Pretreatment Program Elements Review
The Headworks Analysis (HWA) renewal was submitted on 8/17/2022, The HWA last approval date is
10/31/2017,. The next HWA due date is to be determined.
The last Industrial Waste Survey (IWS) was submitted on 11/22/2021. The IWS was approved on
6/16/2022. The next IWS due is to be determined.
The Sewer Use Ordinance (SUO) was approved on 1/3/2012.

The Enforcement Response Plan (ERP) was submitted on 9/13/2019 and approved on 11/19/2019.
The Long -Term Monitoring Plan (LTMP) was submitted on 6/15/2020 and approval on 9/14/2020.
LTMP File Review
The Long -Term Monitoring Plan (LTMP) is being conducted at the proper locations and frequencies.
Industrial User Permit (IUP) File Review
A review of the file for Austin, #IUP 0006 revealed that the monitoring data was well organized and
compliant.
The slug control plan for Austin, #IUP 0006 was in good order and in file.
The Toxic Organic Management plan (TOMP) for Austin, #IUP 0006 was in good order and in file.
Action Items
No other action items were noted.
The pretreatment program is considered satisfactory. The Control Authority runs an excellent pretreatment
