However, additional information is required before the review may be completed. Please address the items on the attached pages no later than the close of business on June 7, 2023. Please be aware that you are responsible for meeting all requirements set forth in North Carolina rules and regulations. Any oversights that occurred in the review of the subject application package are still the Applicant’s responsibility. In addition, any omissions made in responding to the outstanding items in Sections A through E, or failure to provide the additional information on or before the above requested date may result in your application being returned as incomplete pursuant to 15A NCAC 02T .0107(e)(2). Please reference the subject application number when providing the requested information. Application Fee: 1. Not applicable. C. Application (Form: WWIS-R 02-21): 1. The provided coordinates for latitudes and longitudes within the application do not correspond to the locational data in our records, with several of them plotting in locations that do not appear to be associated with the Governor’s Club community. Please review the geospatial data associated with the storage features, monitoring wells, and irrigation sites, and please revise the application accordingly. Please provide data to the sixth decimal degree, if available. 2. Within Section V – Operator in Responsible Charge (ORC), Mr. Eric Riggins is listed as being the designated back-up for Governors Club WWTP. A review of our database indicates that Mr. Eric Riggins is no longer the back-up ORC for this facility (expiration January 1, 2021). Additionally, Mr. Rodney Clark (SI / 1004674) is designated as a back-up, but his status is currently “Invalid”. Please contact the Water Pollution Control System Operators Certification Commission (WPCSOCC) to update the designated operator list to include Mr. Riggins and to resolve Mr. Clark’s status. 3. The existing permit contains several compliance schedules that pertain to the 27,000 gallon aerobic digester modification (Conditions I.1 through I.3). Our office has not received documentation for these conditions at the time of this letter. Please confirm that the 27,000 gallon aerobic digester modification has not been constructed. D. Additional Documentation:  Certificate of Public Convenience and Necessity: 1. No comments.  Site Map: 1. See comment 1.d. in section “Existing Permit” below. 2. Please provide a facility map that delineates the Phase 1, Phase 2, and Phase 3 treatment units. This facility map shall meet the requirements of 15A NCAC 02U .0201(d)(1).  Flow Reduction: 1. No comments.  Property Ownership Documentation: 1. See comments 1.a, 1.b, and 1.c., in section “Existing Permit” below. Condition I.4 of the existing permit issuance dated March 25, 2020, required a permit modification to be submitted within 180 days of that issuance. Our office did not receive a permit modification, and the following items are requested to satisfy this condition: a. A setback waiver for the treatment facility (50 feet to property owned by Governors Club Inc) pursuant to 15A NCAC 02U .0701(a). b. An easement waiver for the Duke/Progress Energy power lines for Storage Ponds A and B. c. An easement for the irrigation fields filed with the Chatham County Register of Deeds that meets the requirements of 15A NCAC 02L .0107(f). d. A scaled map showing the irrigation area wetted perimeter, parcel lines, surface waters, stormwater structures (ditches, inlets, etc.) and groundwater wells (monitoring, potable, etc.) identify the originally permitted 128 acres of irrigation area. Please submit the necessary items to satisfy Condition I.4 of the existing permit. E. Recommendations: (Response not required) 1. The subject facility is classified as a reclaimed water generation and dedicated utilization facility regulated under 15A NCAC 02U and is not classified as a wastewater irrigation system (WWIS) which is regulated under 15A NCAC 02T.
