well as additional information received on September 29, 2006; and October 9, 2006-- we are forwarding
herewith a modified Attachment B, certified October 13, 2006,' to the, City of Monroe for the subject...
residuals land applicationiprogram.
This • certification is being- issued to modify Attachment B with an addition of 1,045 acres to.
already permitted acreage, for a total. of 2,872 acres in Union and Anson counties,"and the. -removal of
field NC-UN-55-12 (13.71 acres). Please note that new maps for the Cox Brothers Far ms'fields, NC -UN
22, 9-12 (previously permitted in another program), will need to be re -mapped within 90 days of -this
certified Attachment B.
Please replace the existing Attachment B of the subject permit that was previously certified `.`.•
on August 23, 2005, with this modified Attachment B. Note .that only the vicinity and buffer maps for
any new and modified land application sites are attached to this modified Attachment B. It is' -'-the
Permittee's responsibility to ensure that a complete permits created byinserting inserting this " .modified
Attachment B and vicinity and buffer maps into Permit No'. WQ0001346, issued on August 23, 2005, and
making sure to retain any'vicinity and buffer -maps for any existing and unmodified land application sites.,
This modified Attachment B shall be , effective from the date of certification;- shall void ' the -
Attachment B that was previously certified on August 23, 2006; and shall be subject to the conditions and -
limitations as -specified therein and well as those specified in the above -referenced permit. As.'always; :.
remember to take the time �to review this modified Attachment B thoroughly, as some of the
conditions contained ther,'ein may have been changed since the last certification. Note that this modified
Attachment B is hereby incorpora±ed •by reference into' Permit No. WQ0001345, issued or. August` 23,
2005.
If any parts, requirements, and/or limitations contained in this certification or modified
Attachment B are unacceptable, you have the right to request an adjudicatory hearing upon. Vvritten
