The limits of disturbance (LOD) for the proposed seepage collection system will be limited to the areas necessary for placement of the seepage
collection sumps and will have no additional impacts to streams. No impacts to the Neuse River or Neuse River Riparian buffers are proposed.
Impacts to jurisdictional stream features will be kept to the smallest footprint practicable. No impacts to jurisdictional wetland areas will result from
project construction.

Silt fencing will be utilized to prevent sediment from entering the stream features. Silt fencing will be installed beyond the limits of adjacent wetland

Bald eagle and RCW are species listed for Wayne County. The Nauss River may provide foraging and perching habitat for bald eagle; however, no

HF Lee Seepage Mitigation Plan Narrative
The following steps constitute the planned seepage mitigation effort to collected and reroute
baseflow, including seepage, from the discharge channel to the west of the HF Lee 1982
(Active) ash pond. Please refer to the design drawings for locations of select construction
components.
1. Erosion and sedimentation control features will be installed, prior to all construction
activities.
2. Sandbags will be placed upstream of the proposed construction area, to dam baseflow
in the channel. Sandbags will also be placed downstream of the proposed construction
area, to restrict the flow of floodwaters from the Neuse River. These sandbag measures
are temporary and will only be required during the proposed construction activities.
3. A trash pump will be installed in the channel upstream of the sandbags and used to
bypass channel baseflow, to the river, during construction.
4. Erosion control measures will be placed across the channel immediately downstream of
the construction area and upstream of the sandbags, prior to the start of construction
activities.
5. A 5'x'5'x5' exterior (4'x4'x4' interior) precast, reinforced concrete box will be installed
within the channel. The box will be constructed with a 6 -inch (height) x 3 -foot (wide)
opening, located immediately below the box lid. The box will be installed such that the
opening faces the upstream direction, with the invert of the opening even with the
channel bottom elevation. The opening is intended to capture baseflow within the
channel. The box lid is approximately 6 inches thick. Therefore, approximately the top
12 inches of the box will be above the channel bottom elevation.
6. Upon installing the concrete box, the remainder of the excavation will be backfilled with
washed stone.
7. A recovery pump or suction hose to an external pump will be installed in the concrete
box to recover inflow and discharge the water to the adjacent ash pond. A second pump
will be on -site as a redundant backup to the primary pump. Each pump will be equipped
with float switches for low level pump -off and normal level pump -on. During storm events
or flooding conditions, the pumps will be allowed to continue to discharge water to the
ash pond.
8. Based on limited baseflow data (120 -180 gallons per minute), a gate valve will be
installed on the pump discharge line to restrict pump discharge to 180 gallons per
minute. The gate valve will control the discharge of water to the ash pond, even during
storm and flooding events. The pumps will be tested to ensure proper operation.
9. Power supply to the pumps will be provided by mobile, diesel, generators, located on the
crest of the ash pond dike.
10. The downstream erosion control measures and sandbags will be removed.
11. The upstream trash pump and sandbags will be removed.
Project Description
Project involves the collecting and rerouting of a portion of the seeps at the HF Lee site from their
current location in a drainage ditch near the ash basin to the Primary Ash Basin. Once they are routed
to the basin we do not expect the basin to discharge. However, if there is a discharge of the basin it is
covered under NPDES permit NC0003417.
Seeps Description
The coordinates of seeps S -6, S -7, S -8, and S -9 are provided below and illustrated in Figure 1.

The analytical data collected from a seep sampling effort is provided in Table 1. Each of the seeps
except for Seep -8 (which was not sampled due to lack of flow) have concentrations exceeding North
Carolina Surface Water Standards, for parameters such as boron, manganese, zinc and arsenic. Duke
Energy is in the process of permitting the seeps through the NPDES program. However, as noted in the
attached EPA letter, Duke Energy has been directed to reroute these seeps back to the Primary Ash
Basin.

Hi folks: As you can see below, we have just received USFWS concurrence that no northern long -eared bats (federally
threatened species) or summer roosting habitat will be effected by the proposed HF Lee seepage collection system and
associated timber removal. You can consider this a No Effect Determination and proceed with the removal of the
timber when the project is scheduled.

Hi Scott,
We do not currently have any records for NLEB in Wayne County, so I think you could make a No Effect
determination and be done.
In the future, you can check our county lists at the web site below - if the species is not on the list, you can
make a note to your file that there is no effect.

Hi John: Hope you are doing well. As I mentioned on my phone message earlier this morning, we have an
urgent project (seepage collection system) that we need to get installed at the HF Lee facility In Goldsboro,
Wayne County NC. In association with this effort, AMEC conducted a northern long -eared bat habitat
assessment on April 29 and determined that the area had low suitability for NLEB due to a lack of snags/cavity
trees and lack of suitable live trees in the small area of designated removal (see attached assessment
report). Only 12 selected trees will be removed to construct an egress/ingress access to the proposed seepage
collection system.
I (we) typically do not request from you, an expedited review, determination, and authorization to proceed for a
project but unfortunately this project need just became apparent. We are please requesting a determination from
you today and that would be much appreciated. Please let me know if you have any questions or need further
information.
