As iequested, please find attached ENG Form 4345 to replace the Pre- Construction Notification Form
submitted for Envision Future, LLC. After review of the request for minimization to less than 0.1 acres,
the project engineers have determined that the (ill required for sewage disposal setbacks, parking. and home
construction would not allow for reduction of the proposed impacts.
