The purpose of this letter is to respond to your letter for Permit # WQCS00350 dated 5-7-20 and
received on 5-13-20. Your letter requested that I address the causes of the SSO, remedial actions, and all
other actions taken to prevent the recurrence of this SSO.
Causes: pump/motor failure that caused the power to back feed to the board and destroyed the relays,
this relay damage caused the other pumps not to work properly.
Remedial Actions: the pump/motor was replaced and the relay situation has been corrected.
Prevent recurrence actions: the electrician redesigned the distribution of power so that one pump/motor
back feeding will not eliminate the other two pumps/motors from functioning properly.
