In response to the referenced Notice of Violaton dated May 7, 2020 and received May 15, 2020
attached please find documentation prepared by Donne West, City of Burlington Water & Sewer
Pipe Maintenance Supervisor and Collection System ORC.
Mr. West's documentation summarize the initial response to the SSO that occurred near Ashton
Park Lane on January 2, 2020 as well as follow-up cleaning and CCTV inspection conducted on
the outfall line on January 7, 2020. This outfall has been placed on our list of higher priority
lines that are cleaned and inspected quarterly in order to help prevent future issues. This SSO
was caused by a grease blockage. If we find this to be an ongoing issue in this area, we will
conduct a targeted educational campaign for the neighborhoods that flow to this line in addition
our bi-annual city-wide efforts.

On May 15, 2020, the City of Burlington received a letter which indicated a notice of violation.
This letter is a written response to this notice of violation that was requested by your office.
The included documentation is an explanation of the events that occurred after the SSO
incident as well as an attached map of outfall affected. I believe, the measures that the city has
implemented after the incident go above the normal procedure. The City of Burlington works
and strives for maintaining their Collection System with the goal of having no SSO incidents.

Response to Notice of Violation
SSO Incident # 202000013
Below are the steps the City of Burlington Water Resources Department has undertaken since
the SSO to minimize the risk of repetitive SSOs. We believe these steps to have been thorough,
exceeding generally expected response action items.
SSO Response Summary
Included with the 5-day report was summary of event. The summary indicated the onsite
findings and immediate actions which were undertaken. This document, titled SSO incident
#202000013 and dated 1-2-2020 is enclosed herewith. The cause of the blockage was
determined to be grease.
Infrastructure Inspection and Action Taken post SSO incident
On 1-7-2020 The Water & Sewer Pipe Maintenance Division mobilized a sewer easement
machine and CCTV crew to inspect the Dogwood acres outfall. From manhole #08611 which is
one manhole 489 feet upstream of the SSO. CCTV inspection was also done from manhole
#08475 which is 393 feet downstream from SSO. The additional downstream section (515 feet)
of the outfall was cleaned to the tie-in at Tucker Street outfall. No additional issues were found.
Enclosed is a copy GIS map of the outfall.
This section of sewer line has been placed on the Quarterly cleaning and inspection sheet to
help assure that no further SSO issues may occur.
Right -of -Way Maintenance
The right-of-way for the Dogwood Acres Outfall is accessible and is normally mowed twice
yearly.
