This letter serves as the official response from Winston-Salem/Forsyth County Utilities to the
North Carolina Department of Environmental Quality, Division of Water Resources Notice of
Intent to Enforce Civil Penalty for the November 3, 2020 sanitary sewer overflow (SSO), Incident
#202002903, located at 4264 Tise Avenue in Winston-Salem. The incident was a result of pipe
failure within the collection system.
On November 3 at 9:30am, a contractor notified Utilities Field Operations of an SSO on the
sewer outfall at 4264 Tise Avenue. There was a break in the pipe where it crossed Leak Fork
Creek and first responders requested a crew with a backhoe. The sewer main was plugged and
a sewer jet/vac truck was deployed to pump until the repair was completed. The SSO was
resolved by 12:30pm. A diffuser was deployed and the spill area was cleaned following
protocol.
WSFC Utilities continues to make progress in minimizing SSOs and has the proper programs in
place to continue our high level of performance. Our comprehensive sanitary sewer evaluation
study has provided strategic cleaning and examination of our system to identify areas with the
greatest need for repair and replacement.
