WHERE THIS IS NOT POSSIBLE, APPROPRIATE PROTECTION SHALL BE INSTALLED. 9. ALL DRAINAGE PIPES AND STRUCTURES SHALL BE CLEANED OF DEBRIS AND ERODED MATERIALS RESULTING FROM CONSTRUCTION. CONTRACTOR SHALL INSTALL AND MAINTAIN EROSION CONTROL MEASURES AS DESCRIBED ON THE PROJECT PLANS AND DETAILS DURING CONSTRUCTION AND SURFACE RESTORATION UNTIL SUCH MEASURES ARE NOT NECESSARY. WHERE ADDITIONAL MEASURES ARE NECESSARY, THE CONTRACTOR SHALL INSTALL AND MAINTAIN THE APPROPRIATE MEASURES ACCORDING TO THE REGIONAL OFFICE OF THE DIVISION OF WATER QUALITY HAVING JURISDICTION. 12. SEED ALL DISTURBED AREAS AND MULCH AS REQUIRED. 13. CONTRACTOR SHALL CONFORM TO ALL APPLICABLE CODES AND OBTAIN APPROVAL AS NECESSARY BEFORE BEGINNING CONSTRUCTION. 17. THE CONTRACTOR SHALL PROVIDE A SMOOTH TRANSITION BETWEEN EXISTING PAVEMENT AND NEW PAVEMENT. SLIGHT FIELD ADJUSTMENT OF FINAL GRADES MAY BE NECESSARY. 18. ALL DEBRIS FROM CHIPPING MACHINES DURING CLEARING OPERATIONS SHALL BE REMOVED FROM THE PROJECT SITE AND DISPOSED OF BY CONTRACTOR. 19. ALL UTILITIES AFFECTED BY CONSTRUCTION ACTIVITIES SHALL BE LOCATED HORIZONTALLY AND VERTICALLY BY THE CONTRACTOR PRIOR TO CONSTRUCTION ACTIVITIES COMMENCING AND ANY POTENTIAL CONFLICTS WITH THE CONSTRUCTION OF THE PROPOSED GAS LINE, BORES, OR OTHER CONSTRUCTION SHALL BE REPORTED TO THE ENGINEER FOR ANY REDESIGN REQUIRED. 20. LAND FARMING OF DRILLING MUD ON THE PIPELINE EASEMENTS WILL BE THE CONTRACTOR'S RESPONSIBILITY TO NEGOTIATE WITH THE INDIVIDUAL PROPERTY OWNERS IN ADVANCE OF SUCH ACTIVITY. SITE NOTES 1. CONTRACTOR SHALL REPAIR ALL DAMAGE TO EXISTING FEATURES (PAVEMENT, CURBS, WALKS, DRIVEWAYS, YARDS, LANDSCAPING, ETC.) TO A LIKE NEW CONDITION AT NO ADDITIONAL COST TO THE OWNER. 2. ALL TRENCHES CUT IN EXISTING ROADS OR DRIVES SHALL UTILIZE A CLEAN SAW CUT AND SHALL BE BACKFILLED (100 %) TO FINAL SUBGRADE ACCORDING TO THE REGIONAL OFFICE OF THE DEPARTMENT OF TRANSPORTATION, HAVING JURISDICTION, REQUIREMENTS. 3. CONCRETE FOR CURBS AND SIDEWALKS SHALL BE 3500 PSI CONCRETE. 4. ANY WORK UNACCEPTABLE TO THE OWNER'S REPRESENTATIVE, TO LOCAL REPRESENTATIVE, OR TO UTILITY REPRESENTATIVE SHALL BE REPAIRED OR REPLACED BY THE CONTRACTOR AT NO ADDITIONAL EXPENSE TO THE OWNER. 5. ALL PAVEMENT MATERIALS AND CONSTRUCTION SHALL CONFORM TO THE REGIONAL OFFICE OF THE DEPARTMENT OF TRANSPORTATION STANDARD DRAWINGS AND SPECIFICATIONS. 6. ALL PAVEMENT STRIPING SHALL BE THERMOPLASTIC OR APPROVED ALTERNATIVE. GRADING NOTES 1. PROPER TEMPORARY AND PERMANENT MEASURES SHALL BE USED TO CONTROL EROSION AND SEDIMENTATION IN ACCORDANCE WITH ALL LOCAL, STATE AND FEDERAL REGULATIONS. STRIP TOPSOIL FROM ALL CUT AND FILL AREAS AND STOCKPILE UPON COMPLETION OF GENERAL GRADING DISTRIBUTE TOP SOIL OVER ALL DISTURBED AREAS, TO A MINIMUM DEPTH OF 6 ", CONTRACTOR SHALL SUPPLY ADDITIONAL TOP SOIL IF INSUFFICIENT QUANTITIES EXIST ON SITE. 8. POSITIVE DRAINAGE SHALL BE ESTABLISHED AS THE FIRST ORDER OF WORK AND SHALL BE MAINTAINED AT ALL TIMES DURING AND AFTER CONSTRUCTION. SOIL SOFTENED BY PERCHED WATER IN FOUNDATION AND PAVEMENT AREAS MUST BE UNDERCUT AND REPLACED WITH SUITABLE FILL MATERIALS APPROVED BY A GEOTECHNICAL ENGINEER. GROUNDWATER INFILTRATION INTO EXCAVATIONS SHOULD BE EXPECTED, AND THE WATER SHALL BE REMOVED USING GRAVITY DRAINAGE OR PUMPING. 9. THE LOCATION OF ALL DIVERSION SWALES AND DITCHES SHALL BE FIELD ADJUSTED TO AVOID TREES AS MUCH AS POSSIBLE. THE CONTRACTOR SHALL WALK THE ALIGNMENT OF THESE SWALES AND DITCHES IN THE FIELD TO VERIFY AVOIDANCE OF TREES. 10. STOCKPILED TOPSOIL OR FILL MATERIAL IS TO BE CONTAINED SO SEDIMENT RUN -OFF WILL NOT CONTAMINATE SURROUNDING AREAS OR ENTER NEARBY STREAMS. 11. ANY SITE USED FOR DISPOSAL AND /OR STOCKPILE OF ANY MATERIAL SHALL BE PROPERLY PERMITTED FOR SUCH ACTIVITY. IT IS THE RESPONSIBILITY OF THE CONTRACTOR TO SEE THAT ALL REQUIRED PERMITS ARE SECURED FOR EACH PROPERTY UTILIZED. A COPY OF THE APPROVED PERMIT MUST BE PROVIDED TO THE INSPECTOR PRIOR TO COMMENCEMENT OF WORK ON ANY PROPERTY. FAILURE TO DO SO MAY RESULT IN THE CONTRACTOR REMOVING ANY ILLEGALLY PLACED MATERIAL AT HIS OWN EXPENSE. 12. AFTER STRIPPING TOPSOIL AT STATION SITES, PROOF ROLL EXPOSED SUBGRADE WITH A LOADED TRUCK TO IDENTIFY UNSTABLE SOILS. STABILIZE UNSTABLE AREAS BY UNDERCUTTING AND REPLACING UNSTABLE MATERIALS WITH COMPACTED SELECT MATERIAL. 13. ALL FILL SHALL BE FREE OF ALL ORGANIC MATTER, TRASH AND OTHER DELETERIOUS MATERIALS, CONTAINING NO ROCK FRAGMENTS GREATER THAN 6 INCHES IN ANY ONE DIRECTION. 14. STRUCTURAL SOIL FILL SHALL HAVE A STANDARD PROCTOR MAXIMUM DRY DENSITY OF 90 POUNDS PER CUBIC FOOT (PCF) OR GREATER AND A PLASTICITY INDEX (PI) OF 30 PERCENT OR LESS. ALL MATERIAL BEING USED AS SOIL FILL SHOULD BE TESTED AND APPROVED BY THE GEOTECHNICAL ENGINEER BEFORE BEING PLACED. 15. PLACE AND COMPACT FILL IN ACCORDANCE WITH SPECIFICATIONS. 16. PROPOSED CONTOURS AND SPOT ELEVATIONS SHOWN ARE FINISH GRADE. PROPER ADJUSTMENT SHOULD BE MADE BY CONTRACTOR FOR SUBGRADE. 17. EXISTING ELEVATIONS, CONTOURS, AND BENCHMARKS SHOWN ON PLANS ARE BASED ON NAD88. 18. SHAPE ALL DITCHES ACCORDING TO TYPICAL SECTION OR SPECIAL DITCH DETAIL. 19. VARIATIONS IN PLAN OR PROFILE 1. THE CONTRACTOR SHALL MAINTAIN THE HORIZONTAL ALIGNMENT (PLAN VIEW) AS INDICATED ON THE PROJECT PLANS WITH A t2 FOOT VARIANCE. 2. CONTROL POINT ELEVATIONS SHOWN INDICATE THE MINIMUM COVER AND SHALL NOT BE REDUCED. THE CONTRACTOR MAY DRILL DEEPER WITH THE APPROVAL OF THE ENGINEER. 3. THE CONTRACTOR MAY REQUEST CHANGES TO THE PROPOSED VERTICAL AND HORIZONTAL ALIGNMENT OF THE INSTALLATION AND THE LOCATION OF THE ENTRY AND EXIT POINTS FOR HDD BORES. PROPOSED CHANGES SHALL BE SUBMITTED TO THE OWNER AND RECEIVE APPROVAL OF THE OWNER PRIOR TO STARTING THE EFFECTED ACTIVITY. BORE PILOT HOLE REQUIREMENTS 1. THE CONTRACTOR SHALL AT ALL TIMES PROVIDE AND MAINTAIN INSTRUMENTATION WHICH WILL ACCURATELY LOCATE THE PILOT HOLE, MEASURE DRILL STRING AXIAL AND TORSION LOADS, AND MEASURE DRILLING FLUID DISCHARGE RATE AND PRESSURE. 2. THE OWNER WILL HAVE ACCESS TO THESE INSTRUMENTS AND THEIR READINGS AT ALL TIMES. 3. THE CONTRACTOR, WHEN REQUESTED BY THE OWNER, WILL PROVIDE INSPECTION HOLES AT NO ADDITIONAL COST TO VISUALLY VERIFY THE LOCATION AND CONDITION OF THE DRILLED PIPELINE. 4. THE CONTRACTOR SHALL PLOT THE ACTUAL HORIZONTAL AND VERTICAL ALIGNMENT OF THE PILOT HOLES AT INTERVALS NOT EXCEEDING 20 FEET, AND CORRESPONDING WITH THE PROJECT STATIONING AND DATUM. 5. THE CONTRACTOR SHALL MAINTAIN AND UPDATE AN "AS- BUILT" PLAN AND PROFILE AS THE PILOT BORE IS ADVANCED. 6. THE CONTRACTOR SHALL GRANT THE OWNER ACCESS TO ALL DATA READOUT PERTAINING TO THE POSITION AND INCLINATION OF THE BORE HEAD. WHEN REQUESTED, THE CONTRACTOR SHALL PROVIDE EXPLANATIONS OF HIS POSITION MONITORING AND STEERING EQUIPMENT, 7.
