The bridge will be replaced by a cored slab bridge with approach slabs. CONTRACT TIM� AND LIQUIDATED DAMAGES: The date of availability for this contract is March 16, 2015. The overall complPtion date for this contract is March 15, 2016. Liquidated damages for this contract are Two Hundred Dollars ($200.00) per calendar day. INTERMEDIATE CONTRACT TIME NUMBER 1 AND LIQUIDATED DAMAGES: The ICT (Intermediate Contract Time) which is basically from the time you close the bridge to the time you open the bridge is March 16, 2015 to December 16, 2015. Liquidated damages for this intermediate contract time are Seven Hundred and Fifty Dollars ($750.00) per calendar day. Start Date: Mr. Kincannon asked when S.T. Wooten anticipates starting worlc on the project. Mr. Mozin�o stated thev are readv to �o on March 16 as soon as the si�ns are up. They are ready to start mill and fill operations beginning with maps one (1) through Four �(4). They plan to skip map five (5) because it's just an overlay. They will catch all of the the mill and fill and then go back to catch Water Street. Mr. Kincannon asked Mr. Mozingo how long he thought it would take and Mr. Mozingo said he was hoping they could get in and out within a weelc or two if everything works out as planned. UTILITIES : Mr. Morin said once the detour is set up, the utility subcontractor will come in and perform that work. Mr. Barnhill confirmed they (Hendrix-Barnhill) will be performing the utility worlc. Mr. Kincannon asked Mr. Barnhill if they had any questions for the utility personneL Mr. Barnhill said it is his understanding that the water line has recently been re-drilled and said they want to be sure they have a good location on it because they will be drilling approximately 10 feet off of that location. Mr. Gerard said it is well marked and permanent signs were supposed to be placed 3 to show the difference from the water to the land. He stated that he would check to be sure it has been done. Mr. Campbell said they have not done anything yet on the sewer side. Mr. Kincannon asked if they anticipate doing anything and Mr. Campbell said as far as he knew NCDOT was doing the boring, which Mr. Kincannon confirmed. Mr. Campbell said he is waiting for someone to call him. Mr. Kincannon replied that we have a two week notification before we actually interrupt service and asked if a two weel� notif cation would be acceptable and Mr. Campbell stated that it would. Mr. Kincannon asked if Mr. Campbell had any questions before he and Mr. Gerard were excused from the meeting. Mr. Campbell said as far as he knows it wi11 involve about two-thirds of his collection system with changing this line over so it will have to be a night time switchover while the system is down because it is force main. Mr. Kincannon asked Mr. Barnhill if they have any issues with that and he said they do not. Since there were no additional questions or comments regarding utilities, Mr. Campbell and Mr. Gerard were excused from the remainder of the meeting. Before being excused, Mr. Kincannon told them if they have any questions while the project is ongoing to contact Mr. Jim Jarvis who would be onsite for the duration of the project and Mr. Marshall Gill who wauld be the project manager. Mr. Kincannon said there seemed to be a discrepancy between the contract and the plans so he wanted to icnow if S.T. Wooten has ordered slabs yet. Mr. Morin said they have not. Mr. Kincannon asked what size slabs they were going to order and Mr. Morin stated the plans show 24-inch. Mr. Kincannon agreed and said the proposal had stated 1 foot 9 inch. Mr. Gill stated the plans are correct. Mr. Morin said he believed he had sent in the detention submittal this week. Mr. Kincannon stated that he has not seen the submittal but had noted there was a discrepancy and just wanted everyone to be sure that the plans are saying two feet which are required so there is no confusion on that. NOTICE TO BIDDERS: The contractor will not close NC 304 until the paving, striping (with thermoplastic pavement markings), and the installation of the snowplowable pavement markers, of the detour have been completed. Mr. Kincannon asked if the contractor foresees anv issues with �ettin� the thermoplastic out and Mr. Mozingo responded that he does not. He said that he will touch base with the paint contractor. Mr. Kincannon agreed and said he doesn't want it to be a hold up. MAJOR CONTRACT AND SPECIALTY ITEMS: There are three major contract items including asphalt concrete surface course, Type �9.5B, Class AA concrete (bridge), and the cored slabs. Again, the contract states 3'-0" x 1'-9" for the cored slabs which is incorrect. It is 3'-0" x 2'-0" as shown on the plans. Specialty items include guardrail, long-life pavement marlcings, utility construction, and erosion control items. L'! FU�L PRICE ADJUSTMENT: There is a fuel price adjustment for asphalt line items. The base index price for Diesel #2 fuel is $2.5028 per gallon. DISADVANTAGED BUSINESS ENTERPRISE (DIVISION�2 Mr. Kincannon stated the DBE goal was 8.0% and met. The contractor rs directed to ensure their DBE's are performing their commercially useful function as the contract states and if you do have to replace DBE's for performance or decertification be sure to follow contract guidance. CHANGES IN THE WORK: Mr. Kincannon stated that if we make any changes that result in the reduction or elimination of work to be performed by a committed DBE, the contractor will not be required to seek additional participation. CERTIFICATIOI�i FOR FED�RAL-AID CONTRACTS: Mr. Kincannon advised that we do have federal aid for this project. Certified payrolls will be required and wage rate interviews will be held for all subcontractors. We want to be sure from the beginning that we are following the wage rates specified in the contract to avoid having to request documentation that could delay monthly pay estimates. Mr. Kincannon asked if there is a preferred date for monthly estimates and Mr. Morin replied the end of each month. Mr. Gill confirmed there are no issues with that schedule. CLEARING AND GRUBBING — METHOD II: Mr. Kincannon stated there is clearing and grubbing, Method II, for this contract and asked what borrow source the contractor anticipates usin�. Mr. Mannin� advised thev will be usin� a � � _ _ Arapahoe Pit (Terry Morris) which is a commercial pit. Mr. Kincannon requested the pit information be submitted to Mr. Gi11. Mr. Kincannon aslced Mr. Gill what submittals he has received thus far. Mr. Gill said we have received the rebar re-design for the cored slab. Mr. Morin stated they have submitted the project schedule, demo plan, crane safety plan, pile hammer and detentioning p1an. SHOULDER AND SLOPE BORROW: Mr. Kincannon stated to be sure the borrow being brought onsite meets the criteria specified on page 49 of the contract for PI and pH and that any variance from that must be approved by NCDOT. T�ZAI'FIC CONTROL: Mr. Kincannon said the contractor is only responsible for onsite traffic control and aslced Mr. Jarvis if he had any comments regarding same. Mr. Evans aslced if Traf�c Services would be providing traffic controL Mr. Kincannon and Mr. Gill replied that NCDOT will provide traffic control for the detour route. Mr. Mozingo added that S.T. Wooten would provide the flagging operation. Mr. Kincannon clarified that would be for the paving but during the bridge construction NCDOT would handle the detour. Mr. Evans stated that normally, unless it is specified in the contract, Traffic Services handled jobsite as well as offsite. Mr. Gill referenced the contract and said NCDOT will be responsible for erection and maintenance of all traffic control devices for the road closure and project detour. The contractor is responsible for all temporary trafi'ic control related to the paving of the detour and shall follow the speci�l provision titled "Temporary Traffic Control". Mr. Evans added he wanted to be sure they are able to get message boards in place approximately one week in advance. STABILIZATION REQUIR�MENTS AND ENVIRONMENTALLY SENSITIVE AREAS: Mr. Kincannon stated the actual bridge project is within an ESA (environmentally sensitflve area) requiring s�ecial procedures. SAFETY F�NCE AND JURISDICTIONAL FLAGGING: Mr. Kincannon stated that we are required to have safety fence and jurisdictional flagging up per the contract. CLEARING AND GRUBBING: Mr. Kincannon said that in accordance with the ESA's requirements, the contractor can provide clearing but not grubbing until immediately prior to grading. Once the grading operations begin, work must progress in a continuous manner until complete. Failure on the part of the contractor to complete anv phase of construction in a continuous manner will result in suspension of the project work. He added that we do not have any groundbreaking until all erosion control measures are up on the entire project. FLOATING TURBIDITY CURTAIN: Mr. Kincannon stated we do have floating turbidity curtain and we need to be sure it is being maintained properly. He added that we have had a few projects recently where the turbidity curtains were not maintained correctly and failure to maintain that will result in no pay for that item. If it is not correct, we will not pay for it. � PERMITS: Mr. Kincannon asked Mr. Ward to discuss any permit issues. Mr. Ward stated there is a 401 Water Quality certiiication that was issued for this project via Water Quality Certification 38�3. There was also a buffer authorization that was issued by D�NR. He aslced if Mr. Johnson has that and he replied he has everything except the buffer authorization. Mr. Ward said that he talked with Mr. Stephen Lane and he will provide that to Mr. Johnson. Mr. Ward reviewed some of the conditions of the permit including Conditions 1, 2, 3, 4, 6, 7, 9, 11, 15, 16, 17 and 21. Mr. Ward aslced if there is an interior bent on this project and it was answered there is one. Mr. Ward stated that he has seen on some projects, when they're pouring for the cap, if it is low in relation. to the water the water came in on a high tide and got into the form. They continued to pour concrete which created a mess and caused high pH levels in the creek. He advised �ontractors to be aware, and if they are pouring concrete and water gets into the form, that water needs to be collected and disposed of rather than discharged back into the creek. Mr. Ward confirmed there are no deck drains on this bridge. Mr. Ward directed that if there are any problems on the project with losing sediment or having spills, etc. they be notiiied within 24 hours. Mr. Johnson discussed information regarding the CAMA permit on behalf of Mr. Stephen Lane. He stated t�ie Division of Coastal Management issued a permit for this project. Mr. Lane's comments included some general comments in addition to some speciiic conditions. He stated that 1) the existing bridge must be removed in its entirety including any relic pilings, 2) measures sufficient to restrain sedimentation and erosion shall be implemented in accordance with BMP's, and 3) all bridge deck drainage shall be captured and discharged outside of jurisdictional waters. Mr. Johnson noted there are specific and general conditions attached to the permit and were provided as handouts. Mr. Johnson asked that anyone having questions make contact with him. Mr. Kincannon stated the bridge demo plan does incorporate some plans to maintain or lceep the debris out of the water at all times. CONCRETE WEARING SURFACE: Mr. Kincannon stated that with regard to concrete wearing surface we need to ensure speciiications are being followed to the letter and ensure the subsurface is done prior to placing concrete. Any variance from the specifications must be approved by IVIr. Kincannon. SUBMITTAL OF WORKING DRAWINGS: Mr. Kincannon reviewed contact information for submittal of working drawings. Strueture related information will go to Paul Lambert and Geotechnical information will go to K.J. Kim or Chris Kreider for review. We have the pile hammer submittal and will get that reviewed. CRANE SAFETY: Mr. Kincannon noted that we also have received a crane safety submittal. % MATERIAL REQIJIREMENTS: In Mr. Milton Rudd's absence, Mr. Metcalfe reminded contractors to comply with the Buy Aineriea certifications and rebar samples will need to be submitted for testing. Mr. Kincannon stated the contractor should be sure any pre-packaged grout is free of soluble chlorides. We have had issues recently where the package indicated that but testing showed that it did contain soluble chlorides. Mr. Metcalfe added, with regard to grouting key ways to be sure and leave a rough surface on the grout and we need to be sure there is a good bond between the wearing surface and the key ways. DECORATIVE METAL RAIL AND CONCRETE PARAPET AND PED�STALS (SPECIAL): Mr. Kincannon noted there is a provision for a decorative metal rail and concrete parapet and pedestals. Mr. Metcalfe asked if those are supposed to match anything or if that has to be discussed with any municipality. Mr. Kincannon responded that has already been done. POWDER COATING OVER GALVNIZING: Mr. Kincannon reminded the contractor to %llow the powder coating provisions in the contract and if there are any questions we would address those with Materials & Tests. PROJECT AESTHETIC LIGHTING: Mr. Kincannon stated that the contractor actually performing the work must have the proper license to perform the work. OTHER ITF,MS DISCUSSED: Mr. Mozingo asked if any erosion control items need to be installed before any paving tal�es place. Mr. Kincannon and Mr. Jarvis agreed they do not believe that is necessary as we do not foresee anv erosion control issues around the detour route and feel the only time we may encounter ✓that is on the actual bridge site. Mr. Jarvis asked who the concrete supplier will be and Mr. Morin advised it will be Greenville Ready Mix (DPD). Mr. Mantell asked who the electrical entity is for this project and Mr. Gill confirmed that will be Dulce Progress. Mr. Mantell asked if NCDOT has a contact for Pamlico County since he will need to meet with them to verify where service will be located. Mr. Gill advised that he has a contact for them and will provide that information to Mr. Mantell. Mr. Frazier asked if the utilities have been relocated and Mr. Gill responded that everything had with the exception of the sewer that is in the contract. With respect to the decorative lights, Mr. Mantell stated they show a receptacle on each pole and asked if this would be maintained by the Town of Bayboro. His reason for asking is that in the past they have run into instances they want the receptacle to operate off the photocell just as the lights will as most o�the time they are just plugging in holiday lights. He added that on one project they ran just one wire for everything but later were told they wanted every other pole to be hot 24/7. He wants to be sure they do it correctly the first time. Mr. Kincannon advised Mr. Mantell that we will get an answer for him. Mr. Frazier posed a question regarding the signing and asked if there is any construction signing that needs to be installed prior to detour work beginning or if moving temporary signs will be sufficient. Mr. �vans stated that NCDOT would take care of all the detour signing. As far as for resurfacing, the contractor will handle the signage. Mr. Frazier asked for clarity if they are responsible for installing construction zone signs and Mr. Mozingo said yes for the paving. Mr. Gill stated that information can be found under Line Item 33. Mr. Metcalfe asked if there are any issues with hauling dirt, hauling cored slabs or the piles on the detour as he wasn't sure if there are any buildings or insufficient shoulders. He added that if working on the east side of the bridge and you're bringing slabs, piles, etc. around there: �s a 90- degree turn. Mr. Evans replied that particular area is pretty wide so there shouldn't be any problem. He added that last year, while resurfacing Water Street, there was a lot of foot traffic in that area. Mr. Kincannon asked if there is enough asphalt that we could widen that area if need be and Mr. Jarvis replied there is not enough shoulder but they should be able to get around the turn. Mr. Morin inquired about construction surveying and said there is a line item for it. Mr. Kincannon asked who the surveyor will be and Mr. Morin advised it will be The East Group. Mr. Kincannon stated that with regard to construction surveying we want to be sure the surveyor talces into consideration the adjacent elevations as they actually are on the ground and not necessarily per the plan and if there is any variation that would require an alternate of grades to let us know as soon as possible. If any grade adjustments need to be made due to a conflict between actual and blanned �rade. we will need to know. Mr. Kincannon added that Rich Godley will be the NCDOT survey party chief and the contact for any verifications or questions regarding that. Mr. Evans asked Mr. Jarvis if, once paving was complete, he saw one and a half to two weeks before they would be ready to close the bridge. Mr. Jarvis adviseoi that once paving was complete, they are to provide two weeks notice. Mr. Evans replied that was good because they would need to get utilities located and message boards set up. Mr. Kincaru�on let Mr. Lvans know we would communicate with him during the process and let him know when they are about to be done. Mr. Mozingo advised they would be doing nuclear density testing. It should be noted that after the preconstruction conference, the decision was made by Mr. Kincannon and Mr. Mozingo to cut core samples instead of nuclear density testing. G� Mr. Mozingo aslced where there is 3-inch milling if there would be somewhere they could transition to 1-1 /2 inch when they reach the bridge deck so the final layer ties in and Mr. Kincannon did not see a problem with that. Mr. Mozingo added they would place some paper joints temporarily until they could get the final lift in since they would be milling 3 inches and putting 1-1 /2 inches bacic in so that one section was going to get paved twice.
