NPDES permit NCO055158 is hereby rescinded, effective immediately. This message is to close the
permit file for the Division, and for your records.
If in the future you wish to discharge wastewater to the State's surface waters, you must first apply
for and receive a new NPDES permit.
PLEASE NOTE: as part of the Division's conversion to paperless communication, this email is the
only notification you will be sent regarding the permit rescission. No hard copy will be mailed.

Good morning, Mr. Weaver. On the behalf of the Town of Bermuda Run, S&ME prepared the appended letter
requesting that NCDEQ rescind NPDES Permit # NC0055158. The Town Engineer (John Grey) and Town
Manager (Andrew Meadwell) reviewed and approved this request. Also appended are documents provided by
Mr. Gary Hudson of the NCDEQ Winston-Salem Regional Office, which support this request.

Dear Mr. Meadwell,
On March 23, 2023, Gary Hudson of this office met with Edmund Henriques, L.G.,
Principal Geologist of the S&ME engineering firm, to perform a site visit and final
inspection to confirm the demolition of the above referenced wastewater treatment plant.
All the WWTP components have been removed and the effluent pipe has been plugged
with a slip plug in the bell end of the 8 inch PVC pipe. It is our understanding that the
entire wastewater flow is now being pumped to the City of Winston-Salem's Muddy Creek
WWTP (NC0050342).
