Accordingly, we are forwarding the attached NPDES discharge permit. This permit is issued pursuant to the requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement between North Carolina and the U.S. Environmental Protection Agency dated October 15, 2007 (or as subsequently amended). This final permit includes no major changes from the draft permit sent to you on December 3, 2008. If any parts, measurement frequencies or sampling requirements contained in this permit are unacceptable to you, you have the right to an adjudicatory hearing upon written request within thirty (30) days following receipt of this letter. This request must be in the form of a written petition, conforming to Chapter 150B of the North Carolina General Statutes, and filed with the Office of Administrative Hearings (6714 Mail Service Center, Raleigh, North Carolina 27699-6714). Unless such demand is made, this decision shall be final and binding. Please note that this permit is not transferable except after notice to the Division. The Division may require modification or revocation and reissuance of the permit. This permit does not affect the legal requirements to obtain other permits which may be required by the Division of Water Quality or permits required by the Division of Land Resources, the Coastal Area Management Act or any other Federal or Local governmental permit that may be required. The rest area provides traveling comforts and amenities to citizens traveling along 1-77 North in Carroll County. The facility is served by a public water supply well and an onsite wastewater treatment plant, which discharges disinfected effluent to a receiving stream in Surry County, North Carolina. The receiving stream is the Naked Run Creek in the Yadkin — Pee Dee River Basin. The North Carolina Department of Environment and Natural Resources, Division of Water Quality has issued a NPDES permit to establish this facility as being acceptable to discharge disinfected effluent to the Naked Run Creek. The NPDES permit number for this facility is NCO039420 and expires on February 28, 2009. The facility has been offline for the beginning of this year, 2008, due to facility renovation. TYPE OF TREATMENT AND TREATMENT REQUIREMENTS The wastewater treatment plant is a secondary treatment process utilizing the extended aeration/activated sludge process. Plant effluent is disinfected utilizing a UV disinfection system and re -aerated prior to discharge to the receiving stream. The UV disinfection system is backed up by an alternate disinfection system which chlorinates and dechlorinates the treated effluent prior to discharge to the receiving stream. Nitrification of the wastewater is also accomplished to meet effluent standard regarding Ammonia Nitrogen (NH3-N). The plant is designed to treat domestic wastes generated from the daily operations at the Carroll County Rest Area. The treatment facilities are capable of handling an average daily flow of 6,667 gallons per day (gpd) through each of three parallel package plants for a total of 20,000 gallons per day, and a total organic load of approximately 50 pounds of BOD5 per day, and 12.5 pounds of ammonia NH3-N per day. B. DESCRIPTION OF PLANT TYPE AND FLOW PATTERN Collection System. Wastewater from the rest area is collected by a sanitary sewer and flows by gravity into the influent of the treatment facility. Treatment System. The wastewater enters the treatment plant at the comminutor where solids are shredded and then flows through the bar screen to prevent remaining solids from entering the surge tank. The surge tank is provided to reduce or equalize peak flows entering the plant and to maintain a nearly constant rate of flow to the treatment process. Two surge pumps deliver the wastewater into the flow control box. The flow control box controls the overflow level across a V-Notch weir thereby regulating the flow. A flow splitter box splits the flow to the three aeration tanks. The flow rate should be set to maintain the average daily flow but in no way should it exceed 15 gallons per minute. The surge tank and aeration tanks are provided with blowers and diffused aerators, which provide the necessary oxygen for biological growth. The aeration tanks are designed to completely mix the tank contents and to provide sufficient oxygen for the biological organisms to grow. The total aeration tank capacity has been increased with the addition of the third aeration tank to provide a detention time of 30 hours and the aeration blowers have also been sized to aid in the nitrification of the wastewater. Nitrification is the process by which the ammonia in the wastewater is converted to nitrites and nitrates. Nitrification will reduce the pollution effects of the effluent on the receiving stream. Overflows from each aeration tank enters a clarifier tank where settled sludge is collected and either returned to the aeration tank or wasted to the sludge holding tank by means of air lift sludge pump. The clarifier is a hopper bottom tank sized for 6 hours detention time. Airlift sludge pumps are sized for 1 1/2 times the design flow. Each of the sludge holding tanks are capable of holding 2000 gallons of sludge. The extended aeration treatment plant is designed to remove the regulated constituents of BOD5, total suspended solids and ammonia nitrogen below the regulated limits. The expected efficiency should be 85% to 95% in order to meet the limitations. The wastewater flows by gravity through the UV disinfection system where the remaining pathogens are disinfected. The UV disinfection system utilizes ultra violet light to alter genetic DNA of bacteria, viruses, mold, algae and other microorganisms. The alteration of the DNA sequence prevents the reproduction process and the pathogens are considered dead; eliminating the risk of disease. The final step in the treatment process is post aeration of the treated wastewater. The flow enters the post aeration tank where the blowers and diffused aeration headers provide the required dissolved oxygen to meet the effluent standards. An ultrasonic flowmeter in the post aeration tank is provided to indicate the instantaneous flow rate, and to record the total flow through the treatment plant. Sludge Handling. The extended aeration treatment process normally results in reduced quantities of sludge to be wasted. Waste sludge is pumped from the clarifier to the sludge holding tank by the airlift pumps. The operator controls the sludge return and waste rates by adjusting the gates within the sludge return trough. The aeration header from the aeration tank is extended into the sludge holding tank to provide oxygen to the sludge and thereby reduce sludge odors. An aerated sludge holding tank is provided on each of the three package treatment plants. Each holding tank is sized at 20% of the aeration tank volume for a combined capacity of 6000 gallons. The extended aeration process does not generate much waste sludge. However, some sludge does require disposal. The operator hauls the excess sludge, which has accumulated in the sludge holding tanks. Sludge is removed whenever the tank becomes full with solids. The wastewater sludge hauler for Virginia Department of Tranportation Rest Area located on I-77 at mile marker 1 is: P.D.
