In accordance with the request for discharge permit application received on December 28, 1998, the
Division is forwarding herewith the subject state - NPDES permit. This permit is issued pursuant to the
requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement between
North Carolina and the U.S. Environmental Protection Agency dated December 6, 1983.

The following modification will be made at the next renewal of the subject permit in June
An effluent limit for total residual chlorine (TRC) of 28 µg/l will be added. North Carolina
has a water quality standard for chlorine that must be applied to all streams with a trout
designation. The receiving stream for the Virginia I-77 Rest Area has a stream classification
of WS-1V Trout. The current levels of chlorine being discharged from your facility average
1510 µg/l and could be toxic to aquatic life in the receiving stream. DWQ procedure requires
that all facilities in trout streams will receive a TRC limit not greater than 28 µg/l. Because
this limit has been inadvertently omitted from the permit, the Division will recommend its
inclusion upon the next renewal. Your permit renewal application indicated that the
treatment facility has a dechlorination unit, therefore the recommended limit for chlorine
should be attainable.
If any parts, measurement frequencies or sampling requirements contained in this permit are
unacceptable to you, you have the right to an adjudicatory hearing upon written request within thirty (30)
days following receipt of this letter. This request must be in the form of a written petition, conforming to
Chapter 150B of the North Carolina General Statutes, and filed with the office of Administrative Hearings,
Post Office Drawer 27447, Raleigh, North Carolina 27611-7447. Unless such a demand is made, this permit
shall be final and binding.
Please take notice that this permit is not transferable. Part II, EA. addresses the requirements to be
followed in case of change in ownership or control of this discharge. This permit does not affect the legal

RESPONSE:
v This agency has reviewed the draft permit and determined that the proposed discharge
will not be sufficiently close to any existing or know proposed public water supply
intake so as to create an adverse effect on water quality. We concur with the issuance
of this permit provided the facility is operated and maintained properly, the stated
effluent limits are met prior to discharge, and the discharge does not contravene the
designated water quality standards.

Virginia DOT I-77 Rest Area is requesting renewal of existing permit. Discharges into
Naked Run Branch, which has a stream class of WS-IV Trout. This is downstream of the
NC DOT 1-77 Rest Area. Naked Run is not on the 303d list for impaired streams in the
Yadkin River Basin. Facility is permitted for 0.020 MGD with limits of:

Facility had a toxicity test -chronic quarterly @ 13% that was dropped in April 1997, via
memo to the permittee.
Review of compliance data indicated that facility now appears to be well operated. It has
met all limits consistently for the past year. There were two exceedences of the NH3
limit in 1997. However, 1996 data shows that the facility had problems meeting TSS,
NH3 and fecal limits from April 1996 to September 1996. Obviously, improvements
were made and the plant is meeting current limits with no problems.
Recommend renewal of the permit with all existing limits. All parameters and
monitoring frequencies remain the same.
8/19/99 Upon finalization of permit, reviewed Chlorine data more closely and found that
Va I-77 Rest Area, has average Cl concentration of 1510 ug/1 in 1998, 1096 ug/l in 1997
and —600 ug/1 in 1996.
Naked Run has a trout designation and a Chlorine standard of 17 ug/l, therefore a
chlorine limit should be assigned. Division policy requires at least a cap limit of 28 ug/l
be given.
Telecon with Jennie Rankin of WSRO and told her that a Cl limit of 28 ug/l would be
added.
Telecon with Paul Szatkowski of Va DOT and told him that Cl limit of 28 ug/l had
inadvertently been omitted from the draft permit. With the facility averaging 1500 ug/l
Cl, I told him that DWQ would give a compliance schedule of 6 months before the Cl
limit would be in effect. Permit info and application both indicate that the treatment plant
has a dechlorination unit. With that unit operating, the Cl limit should be able to be met.
Mr. Szatkowski requested that we send another copy of the draft permit with the new Cl
limit to Ms. Cyndi Ward, Director of Operations, for their review. Told him that the
permit would not be renoticed, but that the revised permit would be issued.

On the basis of thorough staff review and application of Article 21 of Chapter 143, General Statutes of
North Carolina, Public Law 92-500 and other lawful standards and regulations, the North Carolina
Environmental Management Commission proposes to issue a permit to discharge to the persons listed

Richmond, VA 23219 has applied for a permit renewal for a facility located at I-77 Rest Area, Interstate

a permit renewal for a facility located at North Surry High School, NC Hwy 89, west of Mt. Airy, Surry
County. The facility discharges 0.021 MGD of treated domestic wastewater from one outfall into Stewarts
Creek, a Class WS-IV stream in the Yadkin -Pee Dee River Basin which has a 7Q10 flow of 14 cfs. No
parameters are water quality limited, but this discharge may affect future allocations.
3. NPDES No. NC0041939. Surry County Schools, PO Box 364, Dobson, NC 27017 has applied for
a permit renewal for a facility located at I Sam Gentry Middle School, on NC Highway 89, west of Mt.
Airy, Surry County. The facility discharges 0.015 MGD of treated domestic wastewater from one outfall
into an unnamed tributary to Stewarts Creek, a Class WS-11-V CA stream in the Yadkin -Pee Dee River
Basin which has a 7Q10 flow of 0 cfs and a 30Q2 flow of 0.07 cfs. The facility also proposes to relocate
the outfall directly to Stewarts Creek within the next two years. At the current discharge location,
a mmonia and dissolved oxygen are water quality limited. For some parameters, the available load
capacity of the immediate receiving water will be consumed. This may affect future water quality based
effluent limitations for additional dischargers within this portion of the watershed.

Va DOT — I-77 Rest Area is requesting renewal of its existing NPDES
permit. The receiving stream, Naked Run Branch, is not on the 303d list for
impaired streams. Review of compliance data for 1997-1998 indicates no
real problems with plant. Improvements made since 1996 when there were
several violations for TSS and NH3. Instream data does not include DO
monitoring so can't tell if any real problems instream.. Plant appears to be
well operated. Recommend'renewal of existing effluent limits..

Thank you for forwarding the information package to renew the NPDES
permit for our 1-77 NB Rest Area that discharges in Surry County, North Carolina.
The Virginia Department of Transportation (VDOT) wishes to renew this permit.
Since the last permit issuance, some improvements were made to this
facility. VDOT constructed a third aeration tank with a clarifier and sludge
holding tank. This improvement was not intended to increase the plant capacity,
instead, the improvements were made to lengthen the detention time to improve
the quality of our effluent. The North Carolina DEHNR have reviewed and
inspected the plans, O&M manual, and the plant.

Sludge Management Plan Narrative
The wastewater treatment plant at the 1-77NB Rest Area in Surry County
has an extended aeration process that does not generate much waste sludge.
However, some sludge does require disposal. The sludge is confined to a sludge
holding tank.
Sludge is removed by P.D. Quik Sanitary Waste and hauled to the Mt. Airy
sewage treatment plant.
