Please find attached the Stormwater Outfall Discharge Monitoring Reports for Columbia Forest
Products located in Old Fort, North Carolina (Columbia Carolina). Columbia Carolina operates
under Certificate of Coverage Number NCG210136. Samples were collected on 9/6/2022, and
analytical results were received 9/21/2022. Outfall #8 was above benchmarks for COD. A
stormwater inspection was conducted within two weeks and identified the detention basins in the
drainage ditch leading up to Outfall #8 had been filled in with rocks and debris. As a result,
stormwater is no longer collecting in the detention basins, which allow solids to settle out, as
originally designed. This inspection was followed by a Tier I Response Form and appropriate
Corrective Actions, including cleaning or reinstalling the detention basins in the drainage ditch
leading to Outfall #8. Outfall #8 has entered Tier Three with separate correspondence sent to the
Regional Engineer at Energy,Mineral, and Land Resources.
