Upon resubmittal, please upload the actual erosion and sedimentation inspection closeout report issued by NCDEQ-
DEMLR - Raleigh Regional Office. [phone]. Also please download, print out, complete, wet sign, scan and upload
the NOT Certification Form that can be accessed on the eNOT (see blue link within red paragraph). Since Oscar Carmona
is the legally responsible individual, still put his name on the legally responsible person line of the NOT Cert Form, but
then put a note that Conrad Merced is wet signing in his place. Also Conrad Merced prints his name and title on the
bottom of the NOT Cert Form. this also allows Conrad Merced to esign the application. thx
