Dear Mr. Gay:
On April 25, 2023, Brian Lambe from the Wilmington Regional Office of the Division of Energy, Mineral, and Land
Resources (DEMLR), conducted a compliance evaluation inspection for S & W Onslow County Holly Ridge Facility
located in Holly Ridge, Onslow County, North Carolina.
The following observations were noted during the DEMLR inspection (please see the attached addendum for
additional information about your permit):
1) Stormwater Pollution Prevention Plan (SPPP)
A Stormwater Pollution Prevention Plan (SPPP) has been developed, recorded. Additional BMPs are needed to
control dust and offsite sedimentation (from traffic).

Inspection Summary.
Complaint from local resident, excessive dust, sediment, noise
Reevaluate dust control methods. Submit documentation for current BMPs for dust control including methods and frequency.
Provide improvements that may be implemented to remedy the dust issue.
Additional street sweeping is needed. Provide documentation for the current BMPs for offsite sedimentation. You said that
the streets were recently cleaned. Sediment from the sweeping was pushed to the side. Vacuuming of the sediment is
needed.
As stated, provide a response to the complaint with current BMPs and improvements made to in response to the complaint.
Response should be recieved by Monday 5/1/2023.
Analytical monitoring must be electronincally uploaded for this site (and all S & W permit sites). Please contact this office
