Dear Permittee:
The North Carolina Division of Water Resources conducted an inspection of the subject facility on
April 18, 2023. This inspection was conducted to verify that the facility is operating in compliance
with the conditions and limitations specified in Permit No. AWS310395.
As a result of this incident, you are hereby notified that, having been permitted to have a
non -discharge permit for the subject animal waste management system pursuant to 15A NCAC 2T
.1304, you have been found to be in violation of your CAWMP and the Swine Waste System General
Permit No. AWG100000 as follows:
Deficiency 1:
II(2) Failure to maintain adequate cover crop
III(22) Failure to conduct annual sludge survey
III(5) Failure to collect waste sample within 60 days of irrigation event
You are required to provide a written response within 30 days of receipt of this Notice.
Please include in your response all corrective actions already taken and a schedule for completion of
any corrective actions not addressed.
