Dear Mr. Ramey:
Mr. Ron Boone of the Winston-Salem Regional Office of the NC Division of Water Resources (DWR or the Division)
conducted a compliance evaluation inspection (CEI) of the Lissara Wastewater Treatment Plant (WWTP) on April 13, 2015.
This inspection was required as a prerequisite to bringing the plant online for the first time. The assistance and cooperation of
Lee Kimbrell, Operator in Responsible Charge (ORC), during the inspection, was greatly appreciated. The inspection findings
are summarized below and an inspection report is also attached for your records.
The plant is located at 9689 Wildflower Woods Way, in Lewisville, NC, at approximate coordinates 36.116233°N,
80.458243°W. The permit authorizes Lissara Partners LLC to operate this 0.108 MGD WWTP and discharge the treated
effluent from the plant into Yadkin River, this section of which is currently classified as WS-IV waters and is located in the
Yadkin Pee-Dee River basin.
The plant was not yet in operation during this inspection but Lissara Partners LLC have decided that they want to
bring it on line. There are approximately 15 homes now on the system but they feed through filtered septic tanks before being
pumped to the treatment plant. The concern is that the regular daily flow at this point will not provide enough organic material
to establish a viable bacterial population capable of fully treating the waste load.
Mr. Boone and Mr. Kimbrell discussed several operational strategies and adjustments that may help make the plant
successful. Mr. Boone reminded Mr. Kimbrell that exceedences of any of the plants effluent limits may result in violation
notices and ultimately, civil penalties being prepared. Lissara Partners LLC are well aware of the risks of starting up the plant
at this time.
Also, as discussed during the inspection, there is typically a three month delay between the completion of a
Discharge Monitoring Report (DMR) and it being received in our office. Therefore, if violations are experiences within the first
three months of operation it would be prudent to consult with us on whether or not to continue trying to run the plant under the
current waste load.You can contact us at any time.
The plant appears to be in excellent condition and includes all the required components and necessary safety,
security, and redundancy equipment. Documentation required by the permit was discussed and Mr. Kimbrell was in the
process of getting a letter from Lissara Partners LLC enabling him to legally sign the reverse side of the DMRs. You also must

submit the proper form to the Raleigh Central Office and this office to officially designate Mr. Kimbrell as the ORC, as well as
to designate his backup ORC.This paperwork can be found on our website at the following address:

the next 30 days.
Finally, please bear in mind that in accordance with NC General Statute 143-215.6A, the Director of the Division of
Water Resources may assess civil penalties not to exceed $25,000 per day, per violation, for violations of the NC0088749
NPDES permit.
