However, additional information is required before the review may be completed. Please address the items on the attached pages no later than the close of business on March 20, 2023. Please be aware that you are responsible for meeting all requirements set forth in North Carolina rules and regulations. Any oversights that occurred in the review of the subject application package are still the Applicant's responsibility. In addition, any omissions made in responding to the outstanding items in Sections A through M, or failure to provide the additional information on or before the above -requested date may result in your application being returned as incomplete pursuant to 15A NCAC 02T .0107(e)(2). Please reference the subject application number when providing the requested information. Please submit an Operator Designation Form with Mr. Key's information listed. Make sure to follow the directions at the bottom of the form and send it to the WPCSOCC, the Mooresville Regional Office, and the Winston-Salem Regional Office. Please send a copy of this completed form in your response to me as well. After this is completed, Mr. Key will be added as an additional Back-up ORC in our records. A blank Operator Designation Form is included for your convenience. [NCAC 15A 8G .0201] 2. The Mooresville Regional Office has pointed out that the habitable residence located at 400 Old Lowesville Rd., Stanley, NC 28164 (Parcel No. 222965) is less than 400 feet from Field NC-GA- MH-02-05. Please submit: a. An updated buffer map that shows the setback radius for this habitable residence (either 400 feet or a reduced setback). b. A completed Form: LASC 06-16 with only the updated information for Field NC-GA-MH-02- 05. A blank version of this form has been provided for your convenience. c. A signed and notarized setback waiver completed by Mr. Jason A. Edwards that has been recorded in the Gaston County Register of Deeds, which allows reduced setbacks for land application. A blank version of this form with U.S. Biosolids' information filled out has been provided for your convenience. d. If the field acreage will be reduced instead to accommodate the full 400-foot setback, please update Form: LASC 06-16 to reflect the reduced acreage for this field and the permit total; a setback waiver is not required. D. Operation and Maintenance Plan: 1. On the first page of the O&M Plan, a different permit number is listed in the last paragraph. Please revise. 2. On the second page of the O&M Plan in the section where contacts are listed: a. Please revise "...the ORC of Land Application, Town of Dallas and the Winston Regional Office..." to "...the ORC of Land Application, the Town of Dallas, the Mooresville Regional Office, and the Winston-Salem Regional Office...". b. Process Narrative: 1. The Mooresville Regional Office has requested to note on this page that residuals are periodically hauled to the Rocky River WWTP (Permit No. WQ0032332) in Cabarrus County when scheduling and/or weather dictate. The Mooresville Regional Office has commented that they have never met the ORC Dennis Key at any permitted facility or land application site. They recommend that U.S. Biosolids, Inc. reassess the best person for the ORC designation and submit an updated Operator Designation Form as necessary. 2. There is a typo in the word "Immediately" in the first sentence of the spill response plan. The extra breaks on the process narrative sheet make it difficult to follow. Recommend removing this spacing.
