Perkins T.A. Loving Company P.O. Drawer 919 Goldsboro, NC 27533 Subject: Preconstruction Conference Minutes A preconstruction conference for the above referenced projects was held at the Williamston Resident Engineer Office on Thursday, May, 7, 2015. Attendees and Project Personnel: Representing the Prime Contractor, T.A. Loving Company, were Mark Perkins, Clay Wisniewski, and Barry Reeves. Representing the U.S. Army Corps of Engineers was Tracey Wheeler. Representing the N.C. Department of Natural Resources was Garcy Ward. Representing NCDOT were Scott Emory, Michael Hill, Carla Pugh, Clay Willis, John Abel, Michelle Pittman, and Brad Johnson. Permitting Items Discussed: Tracey Wheeler went over the Nationwide 3 Permit. She noted the Special Condition in the permit that states “Floodplain excavation should meet elevation of adjacent wetlands to prevent creation of stagnant water or high ground”. She wants to make sure that there will be no artificial widening of the creek. Contractor’s disposal site is to be determined. Landfills, metal recycle facilities, and/or permitted waste sites will be utilized. They will provide information on which disposal sites will be used. Tracey stated that waste shall not be disposed of in wetlands. Tracey went through the Nationwide Permit Conditions and Regional Conditions. Garcy Ward went through Water Quality Certification No. 3883 conditions. Tracey questioned whether the existing end bents were going to be completely removed. Contractor stated that they would be completely removed. Clay Willis asked if there was a CLOMR on this project. After looking at the plans it was determined that there is not. Clay noted that there shall be not clearing beyond the fill slope. John Abel noted that the power line will be dropped during construction. He asked the contractor to notify Roanoke Electric as soon as the lines could go back up. Right of Way Items Discussed: Michelle Pittman noted that there were no right of way issues. She provided copies of the recorded deeds. Contract Items Discussed: The Contractor has provided progress schedule, list of project personnel, list of material suppliers, list of subcontractors and list of company officials authorized to execute supplemental agreements. Contractor plans to start work on June 15, 2015, which makes the completion date September 22, 2015. John Abel noted that there is currently a steel plate on the existing bridge. He requested that the contractor return that to NCDOT. The Contractor plans to shore up the existing bridge so that they can place a crane on it to begin construction. Scott Emory noted the ICT and the Permanent Vegetation Establishment Provision. The Contractor is familiar with these time restrictions. It was noted that all the steel piles used shall be galvanized from the top of the pile to a minimum of 20 feet below the bottom of the cap. Also, it was noted that an epoxy protective coating should be applied to the tops of all the caps and to the exterior faces of all exterior core slab units. Control points have been provided to the contractor. The Contractor will use 78M stone in the Reinforced Bridge Approach Fills. John Abel noted that there was a waterline installed after the project was designed. He stated that it should not be in conflict with this project. NCDOT will have it marked for the contractor. The Contractor will provide a letter 3 weeks prior to starting construction notifying NCDOT of their start date. Michael Hill brought up that the plans call for concrete collars on the piles. He questioned whether they were actually needed or not since the piles will be galvanized. John Abel verified that the collars are not required. Carla Pugh will meet on site with the contractor at beginning of project to verify that all erosion control is in place and correct. She noted that there is some safety fence required on this project. The Contractor conducts daily safety meetings on site. John Abel stated that the Contractor needs to minimize the amount of time that the power line is down. Contractor will provide NCDOT with a date of when they need the power line dropped. There will be no PDA’s on this project. The contractor has already been provided with driving criteria. This completes the minutes of the Preconstruction Conference for Contract DA00217 held on Thursday, May 7, 2015, at the NCDOT Williamston Resident Engineer Office. Any discrepancies with the content of these minutes should be submitted in writing to this office. Failure to do so within 4 days will be an indication of agreement. Sincerely, C. Brad Johnson, P. E.
