Dear Mr. Shaver:
Thank you for your assistance with this Audubon Villages project for the past couple of
years. We are pleased to submit the following project to you for US Army Corps of Engineers
(COE) 404 approval utilizing Nationwide Permit 18.
We are requesting to place approximately 15 cubic yards into 392 - square feet of
jurisdictional stream areas. This permanent impact is for a twin- culvert stream crossing to allow
for a 41- linear feet driveway crossing. The culverts will be partially submerged to allow for
aquatic migration and endwalls will be utilized to minimize stream impacts. 15- linear feet of
temporary stream disturbance are also proposed for a pedestrian bridge crossing.
We have attached several items to assist your review of the project. If you should have
questions or require additional clarification please feel free to contact me at (919)749 -8259 or
email: jay(aD-kellerenvironmental.com.
Sincerely,

Thanks for speaking with me yesterday Joanne. Attached are the items that were submitted to Raleigh's central office. It's a fairly
simple NWP 18 request for 2 stream crossings and stormwater outlets. The pedestrian crossing will be a bridge but with some
temporary impacts during construction. The driveway crossing is — 3,500SF for a service road. Could you provide us some
correspondence verifying no written authorization required? This will help them with their upcoming ground- breaking ceremony.
Call me with any questions and thanks again for your help. Jay

Hey Jay hope you are well.
I briefly went through the email and its contents and did generate some comments that if addressed may keep the package
from being returned for clarification. Whereas Architectural drawings are nice they are not great permit drawings due to their
clutter. Please assure that the permit drawings are simple showing the jurisdictional boundaries and the placement of the work. The
permit drawings do not need all the topo lines which make it hard to clearly view. Label the impact areas with the same
nomenclature you utilize in the PCN table and label them clearly on the overview plan sheet.
Then move onto the focused plan view with cross section. For the culvert crossing please indicate pipe size, whether or not burial is
proposed (dependent on feature its within stream or wetland), and finally whether temporary impact will be necessary to install the
pipe (typically so if the pipe will be buried).
It looks like both features to be impacted have a dot dash on the JD map which indicates that there may be at least a tributary
system within the jurisdictional boundary. To better characterize the feature I encourage you to rate the system with the DWQ
stream rating form so we have a better idea whether these features have any stream characteristics or not. When I last signed the
map I did not necessarily characterize the features inside the boundary line as that effort should always come during permitting.
Hope this helps and give me a call if you gave any questions,
Brad

Hello Brad & hope you're doing well, it's been awhile. We're submitting a PCN this week for proposed impacts to the linear wetlands
for a driveway & pedestrian crossing. There will be 3,565 SF of permanent (twin culverts) for the driveway & 1,305 SF of temporary
for the pedestrian bridge. I was planning on submitting the following. Please let me know if you need anything else. Thanks again,
