I will grant an extension until Monday, May 25, 2015. However, If the required information in the additional
information letter is not provided by this date then DWR will issue return letter for the project.

Mr. Fox:
This is to follow -up on our telephone conversation concerning the 401/404 PCN for Balsam Mountain Preserve
Community Association Bridge Replacement. The Community Association has indicated that there is some delay in
obtaining a signed Agent Authorization Form from the actual land Owner (i.e., the Developer).
I would like to request a time extension on their behalf.

It is understood that if the signed Agent Authorization Form (or Application Form signed by the actual land Owner)
cannot be provided within the time extension, a new 401/404 PCN application will need to be submitted at a future
date when the proper signatures can be provided.
