In order to meet Condition 6 of the 401 Certification for this project, a stormwater management plan was submitted to the DWQ on August 1, 2007. On August 27, 2007, the DWQ sent you a Stormwater Duplicity of Effort Notice. Since that time, your consultant, John R. McAdams Company, Inc., has demonstrated that this project does not fall under this policy and that the DWQ is the appropriate stormwater reviewing authority for this project. The DWQ has reviewed the stormwater management plan for this project and determined that your stormwater management plan was incomplete as discussed below. Additional Information Requested: The plan details indicate that the stormwater BMP is a stormwater wetland; however, a wet detention basin supplement form was submitted in the stormwater management plan. Please submit a stormwater wetland supplement form if that is the B1VIP that you are intending to use on this site. The current version of the stormwater wetland supplement form can be accessed at the following web site: htt~/1h2o enr.state.nc.us/sta/bmp folms.htm. In your resubmittal, please provide both worksheets of the supplement form (Design Summary and Required Items Checklist) and provide all of the items listed in the Required Items Checklist. 2. Please provide a properly designed forebay that will capture the sediment prior to the stormwater entering the constructed wetland. Plants should not be specified in the forebay in order to maximize sediment storage and facilitate clean out. 3. Please submit a signed and notarized inspection and maintenance form for a stormwater wetland.
