Dear Mr. Chamberlain:
The Division of Water Resources (Division) Winston -Salem Regional Office has reviewed the request for
final closeout of the mitigation projects associated with the construction of the Deep Creek Watershed
Structure 5D, made on your behalf by Mr. Jason Walker with the Yadkin Soil and Water Conservation
District. In addition, In addition, the Division is in receipt of a letter issued by the US Army Corps of
Engineers dated March 26, 2015 regarding this issue. The Division has determined that the mitigation
project has met the success criteria as documented in the 401 Water Quality Certification 1997 -0506 and
monitoring can be terminated at this time.
The long -term protection mechanisms included in the approved Mitigation Plan must remain in effect in
perpetuity. If you have any questions related to our comments or this mitigation project, please feel free
