Does influent sampling apply for a facility that is this small and not required to measure daily flow, i.e. is there a
threshold like there is flow metering (30K)? Since this WWTP is inside a building and pump station is below flood level,
the only place they can sample is from the small EQ compartment/tank. I have confirmed with Wayne Morton that the
tertiary filter(s) backwash is piped through foundation underground to the pump station wetwell, thus EQ will contain a
side -stream which is an issue. Just talked with Matt Hypes and likely 2.5-5 years out. So if we require it, then need to

Also, we have allowed facilities that have switched from private to public go up to nearlya whole permit cycle without
requiring influent sampling. Are we going to require ONWASA's Hickory Grove facultative lagoon facility to perform
influent sampling too it is slated to be taken off-line with Piney Green sewer system expansion)? Not sure how they
would do it at HG as no headworks; sewer pipe exits 30 feet into lagoon perhaps pull manhole lid if not too deep and
hope it has flow at time. Ron has HG (NC0034991) to review — really don't understand why same Permittee is not
assigned to same reviewer.
Is this waterbody impaired/limited for temp whereby this size & type of facility will affect the stream temp? If not, then
this requirement is antiquated and hard to understand when we allow cooling water tower discharges like NCO058271
to only monitor weekly when thermal impact potential is much higher and flow is up to 500K. Seems hard to believe we
are less worried about BOD, TSS, pH, DO, bacti, etc. in the discharge.

would propose the following:
Reduce Temp to weekly measurement.
ONWASA is planning to put a new regional pump station on.Piney Green Rd in front of these apartments and take this
plant off-line within the next 2 years or so. DOT is starting the process to widen the road and is relocating water lines as
we speak with permitting and construction of the Piney Green Wastewater Interceptor system to follow. I would
propose a 24-30 month compliance schedule for implementing influent sampling. I have a call into Matthew about
expected timeframe.

Wow. That's interesting. I'll make the description, address, and lat/long changes in the draft.
Limits:
• Change effluent Temp to weekly versus daily
—Llimits are Water Quality limited, Class II's have to monitor daily.
• Is no Ammonia limit in winter correct? -Correct. Waste Load Allocations dating back to the beginning specifically
say to monitor all year but the limit only applies during the summer.
• At what point does a Total Phosphorus limit get assigned? For past 4 years they consistently run above the 2.0
mg/L level we have at other facilities in this basin. I hope this place will be off-line within next 2-2.5 years, but
who knows with ONWASA.
—This condition was put in the permit in 1997 when it changed from a Class C to a Class C-NSW and added in case
there was a reason to believe that their discharge was causing an impact on the receiving stream.

I actually talked to the Morton's yesterday as a violation showed up and I inquired about it. They asked about their
permit and indicated that last time he talked to Rod Butler apparently ONWASA's attorneys did not want to seek
transfer until after renewal. By the way, Rod Butler resigned from ONWASA on Monday.
That being said, here are my comments.
Facility address is 1906 Piney Green Rd at North Carole Dr, thus drop NCSR 1406
The outfall lat/longs are wrong on the map, but arrow location is OK. Those lat/longs place it on Rocky Run creek. I just
changed them in BIMS and here are the correct ones: Lat 34 44 57 Long 7719 48
System Description Changes:
Add Influent Pump Station (sealed airlift system)
Add Manual Bar Screen
Remove "influent" term from Flow EQ
Add Gas to chlorine disinfection
Add Liquid to dechlorination
Add Post Aeration
Remove "gravity" term from drying beds & replace with "dual"
Limits:

Attached is the draft renewal prepared based on the renewal application provided by Cecil Morton. Please review and
comment if needed.
I know you recommended we hold off on the renewal because it's anticipated that ONWASA will take over this facility.
However, the Director is pressing hard on us to shorten the processing time for renewals so I can't hold off on renewing
the permit and allow it to end up on our backlog.
It shouldn't be a big issue. An ownership/name change request and the necessary documents have to be submitted
regardless of timing. If they can get those submitted prior to the end of the notice period I can do the name change
before the renewal is finalized. If not, it'll go through normal channels here for ownership/name changes.

This is the one ONWASA is supposed to be taking over, thus I would see if we can hold it until they do as the
monitoring frequencies will need to change. I think it was anticipatedtethat ONWASA was going to submit the
change -of -ownership request with the renewal application but their lawyer is holding it up and there may be
some easement filing issues.

A fast track application for Authorization to Construct dechlorination facilities was
received on February 20, 2009, by the Division of Water Quality (Division). Authorization is
hereby granted for the construction of modifications to the existing Webb Apartments WWTP,
with discharge of wastewater effluent into Little Northeast Creek in the White Oak River Basin.
This authorization results in no increase in design or permitted capacity and is awarded
for the construction of the following specific modifications:
Installation of a dechlorination system utilizing calcium thiosulfate pursuant to the
fast track application received on February 20, 2009, and in conformity with the
Minimum Design Criteria for Dechlorination Facilities.
This Authorization to Construct is issued in accordance with Part II1, Paragraph A of
NPDES Permit No. NCO023825 issued August 28, 2007, and shall be subject to revocation
unless the wastewater treatment facilities are constructed in accordance with the conditions and
limitations specified in Permit No. NC0023825.
In the event that the facilities fail to perform satisfactorily, including the creation of
nuisance conditions, the Permittee shall take immediate corrective action, including those as may
be required by this Division, such as the construction of additional or replacement wastewater
treatment or disposal facilities.

The Wilmington Regional Office, telephone number (910) 796-7215 shall be notified at
least forty-eight (48) hours in advance of operation of the installed facilities so that an on site
inspection can be made. Such notification to the regional supervisor shall be made during the
normal office hours from 8:00 a.m. until 5:00 p.m. on Monday through Friday, excluding State
Holidays.
Upon completion of construction and prior to operation of these permitted facilities, the
attached Engineer's Certification form must be received from a professional engineer certifying
that the permitted facility has been installed in accordance with the NPDES Permit and this
Authorization to Construct. Mail the Certification to: Construction Grants & Loans,
DWQ/DENR, 1633 Mail Service Center, Raleigh, NC 27699-1633.
Upon classification of the facility by the Certification Commission, the Permittee shall
employ a certified wastewater treatment plant operator to be in responsible charge (ORC) of the
wastewater treatment facilities. The operator must hold a certificate of the type and grade at least
equivalent to or greater than the classification assigned to the wastewater treatment facilities by
the Certification Commission.
The Permittee must also employ a certified back-up operator of the appropriate type and
grade to comply with the conditions of Tl 5A:8G.0202. The ORC of the facility must visit each
Class I facility at least weekly and each Class II, III and IV facility at least daily, excluding
weekends and holidays, must properly manage the facility, must document daily operation and
maintenance of the facility, and must comply with all other conditions of T15A:8G.0202.
A copy of the approved plans and specifications shall be maintained on file by the
Permittee for the life of the facility.
During the construction of the proposed additions/modifications, the permittee shall
continue to properly maintain and operate the existing wastewater treatment facilities at all times,
and in such a manner, as necessary to comply with the effluent limits specified in the NPDES
Permit.
You are reminded that it is mandatory for the project to be constructed in accordance
with the North Carolina Sedimentation Pollution Control Act, and, when applicable, the North
Carolina Dam Safety Act. In addition, the specifications must clearly state what the contractor's
responsibilities shall be in complying with these Acts.
Prior to entering into any contract(s) for construction, the recipient must have obtained all
applicable permits from the State.
Failure to abide by the requirements contained in this Authorization to Construct may
subject the Permittee to an enforcement action by the Division in accordance with North
Carolina General Statute 143-215.6A to 143-215.6C.

The issuance of this Authorization to Construct does not preclude the Permittee from
complying with any and all statutes, rules, regulations, or ordinances which may be imposed by
other government agencies (local, state, and federal) which have jurisdiction.

permit. Accordingly, we are forwarding the attached final NPDES discharge permit. This permit is
issued pursuant to the requirements of North Carolina General Statute 143-215.1 and the
Memorandum of Agreement between North Carolina and the U.S. Environmental Protection
Agency dated May 9, 1994 (or as subsequently amended).
If any parts, measurement frequencies or sampling requirements contained in this permit are
unacceptable to you, you have the right to an adjudicatory hearing upon written request within
thirty (30) days following receipt of this letter. This request must be in the form of a written
petition, conforming to Chapter 150B of the North Carolina General Statutes, and filed with the
Office of Administrative Hearings (6714 Mail Service Center, Raleigh, North Carolina 27699-
6714). Unless such demand is made, this permit shall be final and binding.
Please take notice that this permit is not transferable. This permit does not affect the legal
requirements to obtain other permits which may be required by the Division of Water Quality or

c) Describe receiving stream features and pertinent downstream uses: Navigable waterway,
uses include fishing. According to the White Oak Basin plan (September 2001), Little
Northeast Creek (8.3 mile stretch) was considered impaired and listed on the 303(d) list
because of low DO recorded at the monitoring station on Little Northeast Creek. Although
fish tissue samples were not collected in Little Northeast Creek, it is considered partially
supporting for the fish consumption use support category because of a statewide fish
consumption advisory for bowfin. DWQ biologists indicated that the benthic
macroinvertebrate community in this creek was healthy, the criteria for evaluating benthic
macroinvertebrate communities in swampy waters is currently in draft form. Webb
Apartments (0.025 MGD), Hickory Grove (0.0225 MGD), Morton Elementary School
(0.0075 MGD), and Horse Creek Farms (0.10 MGD) all discharge to Little Northeast Creek.
The primary source for pollutants has been Hickory Grove with 17 months of violations
including Fecal Coliform, TSS, residual chlorine, Flow, DO, and BOD-5 day. Morton
Elementary has had 3 months of violations including flow, ammonia nitrogen, TSS and total
residual chlorine. Horse Creek had one month in June 2006 showing violations for total
nitrogen limits and Webb Apartments with no violations in the past permit cycle. This
segment of the White Oak basin is among the most heavily populated. The basinwide plan
recommends the point source dischargers continue to seek non discharge options. Given the
location of the facilities and lack of available land in close proximity to these developments,
non discharge options are not likely. Upgrading to BAT limitations will be the only means to
improve water quality as it relates to these dischargers. Currently, per the White Oak
Basinwide Plan, unless the discharge is above 0.05 MGD, no phosphorous limits are
imposed. Only new or expanding facilities will receive TP limits of 0.5 mg/L. Unless the
discharge is above 1 MGD, no total nitrogen limits are imposed; only monitoring for TN and
TP would be required of any of these facilities.
PART II - BACKGROUND AND HISTORY
Above ground 0.025 MGD extended aeration package plant. 100% domestic
wastewater. The owner of the facility is the ORC as well. Being an indoor facility, the
plant is in relatively good condition from a structural integrity standpoint. Corrosion
has been a problem in the past, particularly with the tertiary filters due to chlorination
in the clarifier clear well. Tertiary filters have been rebuilt recently to replace corroded
components.
PART III - DESCRIPTION OF DISCHARGE AND TREATMENT WORKS

WWTP from their permit with no prior warning last year. This occurred as well in 1999,
with no prior warning. Webb Apartments disposes of dry solids from drying beds in the
Onslow County Landfill (10 tons/year).

PART V - EVALUATION AND RECOMMENDATIONS
The equalization basin is located underground next to the influent wet well. When wastewater
exceeds wet well capacity it overflows into the underground equalization basin. The wet well
utilizes air lift to pump wastewater from the wetwell to the aeration basin. Two (2) blowers with 5
HP motors set for alternating use provide the air lift for the wet well. The aeration basin utilizes 2
blowers with 3 HP motors to provide sufficient air. These blowers also provide air to sludge
digester. All blowers and motors are operational. The clarifier has center feed for influent and
peripheral effluent weirs. Gaseous chlorination is utilized for disinfection. The chlorine gas is
introduced into the clarifier clear well prior to entering the two high rate tertiary filters by gravity
feed. The clarifier weir is not even allowing some short circuiting. The ability to readjust the weir is
limited due to the worn condition of the spring assembly on the down rods that support the weir.
The use of tertiary filters helps provide a better effluent quality. The tertiary high rate filters are both
operational and appear to have adequate air scour as well as back wash capability. The filters have
recently been refurbished as requested by the Regional Office pursuant to a wastewater compliance
evaluation inspection conducted in 2005. The effluent from the tertiary filters enters the effluent
well by gravity feed. An immediate drop in elevation from the tertiary filters to the effluent well of
approximately 4 feet provides adequate air to meet the DO requirements specified in the permit. The
effluent flow is measured using an ultrasonic flow measurement devise and chart recorder is
provided. The effluent leaves by gravity to the receiving stream. Two drying beds are located under
a covered roof next to the building housing the wastewater treatment plant. Although it does not
allow the sun to help with the drying process, the restriction of rain on the drying beds is an
advantage.
The Wilmington Regional Office recommends reissuance of the permit in accordance with the White
Oak River Basinwide permitting strategy, provided no significant adverse public comment is
received. The Wilmington Regional Office also requests that the recommendations provided herein
are considered in the reissuance of Permit No. NC0023825.

Division personnel have reviewed and approved your application for renewal of the subject permit.
Accordingly, we are forwarding the attached NPDES discharge permit. This permit is issued pursuant to the
requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement between North
Carolina and the U.S. Environmental Protection Agency dated May 9, 1994 (or as subsequently amended).
This final permit includes no major changes from the draft permit sent to you on May 29, 2002.
If any parts, measurement frequencies or sampling requirements contained in this permit are unacceptable to
you, you have the right to an adjudicatory hearing upon written request within thirty (30) days following receipt of
this letter. This request must be in the form of a written petition, conforming to Chapter 150B of the North Carolina
General Statutes, and filed with the Office of Administrative Hearings (6714 Mail Service Center, Raleigh, North

receiving stream.

Onslow County discharging treated wastewater into an unnamed tributary to Mott Creek in the White Oak
River Basin. Currently BOD, ammonia nitrogen, total phosphorus and total residual chlorine are water quality
limited. This discharge may affect future allocations in this portion of the receiving stream.
NPDES Permit Number NC0036676. Rexon, LTD, (Collins Estate MHP WWTP), 1138 Rocky Run Road,
Apartment 2, Midway Park, NC 28544 has applied for a permit renewal for a facility located in Onslow
County discharging treated wastewater into an unnamed tributary to Rocky Run in the White Oak River
Basin. Currently BOD and ammonia nitrogen are water quality limited. This discharge may affect future
allocations in this portion of the receiving stream.
Written comments regarding the proposed permit will be accepted until 30 days after the publish date of this
notice. All comments received prior to that date are considered in the final determinations regarding the
proposed permit. The Director of the NC Division of Water Quality may decide to hold a public meeting for
the proposed permit should the Division receive a significant degree of public interest.
Copies of the draft permit and other supporting information on file used to determine conditions present in
the draft permit are available upon request and payment of the costs of reproduction. Mail comments and/or

The sludge from the above referenced Plant is dried in dry beds and disposed
of in the Onslow County Landfill, as per letter from Mr. James E. Horne,
Solid Waste Coordinator.
Also, if we have any additional sludge to dispose of that is not dried in dry
beds, Lewis Farms of Burgaw picks this up. He disposes of this under his
permit.
If you need any additional information concerning the Sludge Management
