1. Application (Original Comments 2&5) - I have not yet received the revised Application. The
permit cannot be issued until this information is received. It is noted that the response to
comments indicated that the Application was "sent to owner" however, we have not received
the revised Application.
Response: The application has been revised and hand delivered to you.
2. Original Comment 4, Wet Pond Page, Line 44 —Per Wet Pond MDC 5b, the forebay entrance
must be deeper than the forebay exit (Lines 43 & 44 indicate that the entrance and exit are at
the same elevations). Please revise the plans and/or the Supplement-EZ Form as needed.
Response: The Plans and Supplement-EZ Form has been revised per your request.
3. Electronic files from the 1/17/23 submission were not uploaded to the online file repository
(https://edocs.deg.nc.gov/Forms/SW-Supplemental-Upload).
