Good afternoon All,
In accordance with the attached General Certification #4501 (GC 4501), the impacts described in
your minor modification request to the existing CAMA major permit (DCM131-06) do not require
written authorization to utilize GC 4501. However, you are required to follow the conditions listed in
the attached certification. If the conditions of the attached general water quality certification
cannot be met or the project scope changes so that the thresholds listed are exceeded, additional
review will be required by the Division of Water Resources and an Individual Water Certification may
be required. The approval does not alleviate the need to comply with any other federal, state and
local requirements before proceed with your project.
The Division of Water resources has assigned the project# 20060895. Any future DWR
correspondence should reference this project number. Please feel free to contact me if you have
any questions or need my assistance in any way.

Morning all,
After discussions with the agent there seems to be an addition to the previously submitted request.
On the uplands there is an expansion to footprint of the buildings marked as proposed.
Christine —This was delt with during the SW review and the applicant has received a permit (above)
that was recently provided to DCM.

Dan —The increase mayor may not be over existing impervious. My plan, since this was identified
late, would be to add a not saying the request MAY require a E&S permit. Please let me know if this
works for you.
Maria and Jimmy —should not change your comments since all upland.
Rachel — Let me know if this changes your comments.
Liz — Let me know if you have any needs to complete review.
