De�artmeni of Transportation, 113 Airport Road, Edenton, NC 27932 __ Authorizing development in Bertie County at Cashie River, SR 1500, San Souci Ferry Ram�ISan Souci Road , as requested in the permittee's application dated 2127I15, in�luding the attached workplan drawin�s (4) dated as received on 2/27/15. . This permit, issued on 4I2911 � .__ , is subject to campliance with the application (where consistent with the permit), all applicable regulations, special conditions and notes set forth below. Any violation of these terms may be subject tv fines, imprisonment or civit action; or may cause the permit to be null and void. 1} Unless specifically altered herein, this minor modification authoriz�s a revised in water work moratorium period, a revised area of riprap placement, and the installation of 851inear feet of sheet piling, as depicted on tk�e attached workplan drawings and as described in the permit modification request. 2) In order to protect anadromous fishery resources, no in water work shail oecur between February 15th to ]une lsf of any year without prior appraval of the N.C. Division of Coastal Management (DCM), in consultation with the U.S. Army Corps of Engineers. NOTE: This minor modification wil� decrease the permanent wetland irnpacts by approximately 150 square feet to a total of approximately 2$0 square fee� of impacts to 404 wetlands. (See attached sheet for Additional Conditions) This permit action may be appealed by the permittee or other qualified persons within twenty {20) days of the issuing c�ate. This permit must be accessible on-site to Department personnei when the project is inspected for compliance. Any maintenance work or project modification not covered hereunder requires further Division approval. All work must cease when the permit expires an December 31, 201$ In issuing this permit, the State of North Carolina agrees that your project is consistent with the North Carolina Coastal Management Program. Signature af Permittee Department of Transpartation ADDITIONAL CONDITIONS Perrnit #$-15 Page 2 of 2 3) The alignment of the authorized sheet piling shalI be staked by the permittee and approved by a representative of DCM within a maximum of 30 days prior to the start of any construction. Failure to initiate construction within 30 days, or erosion of the sl�oreline by adverse weather conditions shall require the alignment to be restaked by the permittee and re-approved by DCM within a maximum of 30 days prior to the new expected start of construction. �i 5} The elevation of the top of the sheet piling when installation is comp�ete shall not exceed the elevation of normal water lev�l. In areas where placement of sheet piling and riprap is authorized, the sheet piiing shall be instaIled prior to any placement of riprap. 5) If it is determined that additianal perxnanent andlor temporary impacts are necessary that are not shawn on the attached workplac� drawings or described in the authorized permit application, a permit modification and/or a�ditional authorization fro�n DCM may be required. In addition, any changes in the appraved plan may al�o require a permit modification and/or addrtional authorization from DCM. The permittee shall contact a representative of DCM prior to comz-nencement of any such activity for this determination and any permit modification. 7) This minor modification shal] be attached to the original of Permit No. 5-15, which was issued on 1129115, and copies of both documents shall be readily available on site when a Division representative inspects the praject for compliance. 8) Al1 conditions and stipulations of the active permit remain in farce under this minor modification unless altered herein. NOTE: NOTE: This minor modification does not eliminate ti�e need to obtain any additional perrnits, approvals or authorizations that may be required. A minor madification application processing fee of $100 was received hy DCM far this project
