reactivity (if a TCLP was performed), and metals.
c. Calculations that are within the required limits for SAR.

1. Site 2009 —
a. Please add Fields 1-3 under Section III on Page 2. There is a well next to these fields within
the property boundary. The LOA does not have to be resigned or re -notarized.
2. Site JC —
a. On the LOA for Fields 1-4, please add Field 3 under Section III on Page 2. There is a well next
to Field 3 within the property boundary. The LOA does not have to be resigned or re -notarized.
b. On the LOA for Barbara W. Sykes, please add Fields 6 and 7. The latest property boundaries
show a portion of these fields on Ms. Sykes' property. The LOA does not have to be resigned
or re -notarized.

M. Setback Waiver Aueement(s):
If a setback waiver will be obtained from a property owner for reduced setbacks to habitable
residences and/or property lines, please provide a completed Form: NDRMPW-LAS 11-18, which
can be downloaded from the Non -Discharge website. Please also make sure to update the
associated buffers on the applicable vicinity/setback map. For habitable residences, please notate
the off -site dwelling on the applicable vicinity/setback map to show that the off -site dwelling has
an associated setback waiver.
N. Vicinity/Setback Map(s):
1. Please provide color original or lst generation copies of all vicinity/setback maps as per the policy
document Map Guidance for Residuals Land Application Fields.
2. Site MP —
a. The 400-foot habitable residence setback is not met for Charles M. Phillips' property located
at 1522 Bethlehem Church Road (Parcel No. 035095) to Fields 1 and 14. Please either show
the 400-foot setback buffer and reduce the field acreages accordingly or obtain a setback waiver
from Mr. Phillips. See Section N.
b. The 400-foot habitable residence setback is not met for the property located at 1532 Bethlehem
Church Road (Parcel No. 003617) to Fields 1 and 14. Please either show the 400-foot setback
buffer and reduce the field acreages accordingly or obtain a setback waiver from the property
owner. See Section N.
c. The 400-foot habitable residence setback is not met for the property located at 1543 Bethlehem
Church Road (Parcel No. 007084) to Field 14. Please either show the 400-foot setback buffer
and reduce the field acreage accordingly or obtain a setback waiver from the property owner.

3. Site 2009 —
a. There are updated property boundaries shown on the Granville County GIS portal located
around the western portion of Field 2. Please update the map to reflect the latest property
boundaries including any affected field boundaries, buffer boundaries, application area
boundaries, etc.
b. There are updated property boundaries shown on the Granville County GIS portal located
around the western portion of Field 4. Please update the map to reflect the latest property
boundaries including any affected field boundaries, buffer boundaries, application area
boundaries, etc.
4. Site 375 —
a. Please remove the labels, buffers, and application boundary areas for Field 3 since it will be
removed from this permit. Please also revise the title block of the map to reflect this.
5. Site GF —
a. Is the entire boundary of Field 6 shown on the map view?

6. Site JTB —
a. There is a duplicate "Field 2" label. Please revise.
7. Site BC —
a. There are application boundaries shown within buffer boundaries for all fields. Please revise
as it is unclear where the land application is occurring.
b. Comparing recent aerial imagery to what was shown on the Field 4 map, there appear to be
trees that have been cleared within the field boundaries. Please show the most recent aerial
imagery.
8. Site 8015-01 —
a. Please remove the labels, buffers, and application boundary areas for Field 1 since it will be
removed from this permit. Please also revise the title block of the map to reflect this.
9. Site JC —
a. There are updated property boundaries shown on the Nash County GIS portal located between
Fields 3, 5, 6, 7, and 8. Please update the map to reflect the latest property boundaries. Any
property boundaries with differing owners on each side will either need a 50-foot property line
setback buffer or a setback waiver from each owner. See Section N. If any buffer areas will
be converted to application areas, please provide an abbreviated soil report by a licensed soil
scientist that certifies the previously buffered areas are consistent with the soil type of the
current field application area and are suitable for residual land application. No additional soil
borings or laboratory analyses are needed.
b. The 400-foot habitable residence setback is not met for the property located at 3413 Bass Road
(Parcel No. 009353) to Field 5. Please either show the 400-foot setback buffer and reduce the
field acreages accordingly or obtain a setback waiver from the property owner. See Section N.
10. Site LF
a. Comparing recent aerial imagery to what was shown on the Field D map, there appears to be a
new habitable residence within a separate parcel just east of the field boundary. Please update
the map to reflect the latest aerial imagery and property boundaries and mark the off -site
dwelling. Please either show the 400-foot setback buffer and reduce the field acreage
accordingly or obtain a setback waiver from the property owner. See Section N.
P. Updated Cumulative Pollutant Loading Rates (CPLRs):
1. Please provide updated CPLR data for Site GF Fields 7A and 12A.
Q. Recommendations (Response not required):
1. Outdated forms were used throughout the submitted application package. For future submittals,
please make sure to download the latest forms from the Non -Discharge website before starting the
application process.
2. There is a typo in the word "Control" on one of the divider pages in the O&M Plan.

3. There is a typo in the word "Assessment" in Section 14.0 of the O&M Plan.
4. There is a typo in the "Kerr Lake Regional WTP" label on Figure 15.
5. A TCLP analysis was provided in the submitted application package for Tar River WTP and Sunset
Avenue WTP. Per the existing permit, these are not required to be submitted and can be omitted
from future application packages and responses.
6. There is extra text copied from Page 1 on Page 2 of the Site 4801-WP LOA.
7. The fields listed below have buffers shown that are not required based on a review of property
ownership. If any buffers will be removed, please provide an abbreviated soil report by a licensed
soil scientist that certifies the previously buffered areas are consistent with the soil type of the
current field application area and are suitable for residual land application. No additional soil
borings or laboratory analyses are needed.
a. For Site 4801-JH Field 8, there is a 400-foot habitable residence setback shown for an on -site
dwelling at the southern edge of the field. This is not required for on -site dwellings and can be
removed to increase the application area of the field if this is a confirmed habitable residence.
b. For Site JC Fields 1, 3, and 4, there is a 50-foot property line setback shown on the interior
boundary. This is not required because the same landowner owns all four parcels that contain
these three fields and can be removed to increase the application area of the field.
c. For Site JC Fields 5, 6, and 8, there are three 400-foot habitable residence setbacks shown for
the properties located at 3303 Bass Road (Parcel No. 009305), 3261 Bass Road (Parcel No.
009347), and 3119 Bass Road (Parcel No. 037299). These are not required because they are
considered on -site dwellings. The same landowner owns all or a portion of the buffered fields
that these properties are adjacent to. These buffers can be removed to increase the application
area of the fields.
