On March 10, 2015, the Division of Water Resources (Division) received your application
requesting a 401 Water Quality Certification from the Division for the subject project. The Division
has determined that your application is incomplete and cannot be processed. The application is
on -hold until all of the following information is received:
1. Application Form was not Signed
Page 11 of 11 of the Pre - Construction Notification (PCN) Form was not signed. Pursuant to
15A NCAC 02H 0502(f) the application form must be signed by the owner or the owner's
agent. If an agent of the owner signs the application form, an agent authorization form
must also be included. An example agent authorization form can be found online:

2. Need more copies of the application
Pursuant to 15A NCAC 02H 0502 and G.S. 143- 215.3D(e), the Division requires four (4)
complete application packages, to begin the proper consideration of an application. We
received 1 copy of the application, however we require 4 copies of the application. Please
submit 3 additional copies of the application.
Pursuant to Title 15A NCAC 02H .0502(e), the applicant shall furnish all of the above requested
information for the proper consideration of the application. If all of the requested information is
not received in writing at the address below within 30 calendar days of receipt of this letter, the
Division will be unable to process the application and it will be returned. The return of this project
will necessitate reapplication to the Division for approval, including a complete application
package and the appropriate fee.
