Dear Mr. Boyd:
A fast track application for Authorization to Construct dechlorination facilities was
received on November 2, 2005, by the Division. Authorization is hereby granted for the
construction of modifications to the existing Yadkinville WTP, with discharge of filter backwash
water into South Deep Creek in the Yadkin-Pee Dee River Basin.
This authorization results in no increase in design or permitted capacity and is awarded
for the construction of the following specific modifications:
Installation of a sodium bisulfate dechlorination system pursuant to the fast track
application received on November 2, 2005, and in conformity with the Minimum
Design Criteria for Dechlorination Facilities.
This Authorization to Construct is issued in accordance with Part III, Paragraph A of
NPDES Permit No. NC0079260 issued May 1, 2005, and shall be subject to revocation unless
the wastewater treatment facilities are constructed in accordance with the conditions and
limitations specified in Permit No. NC0079260.
In the event that the facilities fail to perform satisfactorily, including the creation of
nuisance conditions, the Permittee shall take immediate corrective action, including those as may
be required by this Division, such as the construction of additional or replacement wastewater
treatment or disposal facilities.
The Winston-Salem Regional Office, telephone number(336) 771-4600, shall be notified
at least forty-eight (48) hours in advance of operation of the installed facilities so that an on site

inspection can be made. Such notification to the regional supervisor shall be made during the
normal office hours from 8:00 a.m. until 5:00 p.m. on Monday through Friday, excluding State
Holidays.
Pursuant to 15A NCAC 2H .0140, upon completion of construction and prior to operation of
these permitted facilities,the completed Engineering Certification form attached to this permit shall be
submitted to the address provided on the form.
Upon classification of the facility by the Certification Commission, the Permittee shall
employ a certified wastewater treatment plant operator to be in responsible charge (ORC) of the
wastewater treatment facilities. The operator must hold a certificate of the type and grade at least
equivalent to or greater than the classification assigned to the wastewater treatment facilities by
the Certification Commission.
The Permittee must also employ a certified back-up operator of the appropriate type and
grade to comply with the conditions of T l 5A:8G.0202. The ORC of the facility must visit each
Class I facility at least weekly and each Class II, III and IV facility at least daily, excluding
weekends and holidays, must properly manage the facility, must document daily operation and
maintenance of the facility, and must comply with all other conditions of T 15A:8G.0202.
A copy of the approved plans and specifications shall be maintained on file by the
Permittee for the life of the facility.
During the construction of the proposed additions/modifications,the permittee shall
continue to properly maintain and operate the existing wastewater treatment facilities at all times,
and in such a manner, as necessary to comply with the effluent limits specified in the NPDES
Permit.
You are reminded that it is mandatory for the project to be constructed in accordance
with the North Carolina Sedimentation Pollution Control Act, and, when applicable, the North
Carolina Dam Safety Act. In addition, the specifications must clearly state what the contractor's
responsibilities shall be in complying with these Acts.
Failure to abide by the requirements contained in this Authorization to Construct may
subject the Permittee to an enforcement action by the Division of Water Quality in accordance
with North Carolina General Statute 143-215.6A to 143-215.6C.
The issuance of this Authorization to Construct does not preclude the Permittee from
complying with any and all statutes, rules, regulations, or ordinances which may be imposed by
other government agencies (local, state, and federal) which have jurisdiction.

Division personnel have reviewed and approved your application for renewal of the subject permit.
Accordingly,we are forwarding the attached NPDES discharge permit. This permit is issued pursuant to the
requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement between North
Carolina and the U.S. Environmental Protection Agency dated May 9, 1994 (or as subsequently amended).
This permit includes the following changes from the draft permit sent to you on January 7,2004:
➢ The receiving stream classification has been changed from WS-III CA to C.
We cannot grant your request to remove the total residual chlorine (TRC) limit from the permit. Since your
facility uses chlorinated water to backwash the filters,you are adding chlorine to your treatment process. Effluent
data shows consistent discharge of TRC. If you wish to explore dechlorination options in order to meet the TRC
limit,consult the attached guidance document.
If any parts,measurement frequencies or sampling requirements contained in this permit are unacceptable to
you,you have the right to an adjudicatory hearing upon written request within thirty(30) days following receipt of
this letter. This request must be in the form of a written petition,conforming to Chapter 150B of the North Carolina
General Statutes,and filed with the Office of Administrative Hearings (6714 Mail Service Center,Raleigh,North
Carolina 27699-6714). Unless such demand is made,this decision shall be final and binding.
Please note that this permit is not transferable except after notice to the Division. The Division may require
modification or revocation and reissuance of the permit. This permit does not affect the legal requirements to obtain
other permits which may be required by the Division of Water Quality or permits required by the Division of Land
Resources, the Coastal Area Management Act or any other Federal or Local governmental permit that may be

Dear Mr.Weaver:
The Town of Yadkinville received a copy of the draft permit to discharge filter-backwash wastewater
from the Yadkinville Water Treatment Plant into South Deep Creek. NPDES#NC0079260.
Upon review of the attached TRC policy, the Town of Yadkinville feels that a limit of 27 ug/1 is not
needed due to the fact that the town's grade 1 physical-chemical wastewater plant does not add
chlorine or any other disinfectant to the wastewater being discharged.
Please find attached a copy of the TRC policy for NPDES permits. The sentence that the Town of
Yadkinville feels applies to the above is highlighted.

SUBJECT: Total Residual Chlorine Policy for NPDES permits
Every three years the State is required by the Clean Water Act to review its surface water quality standards and
classifications,determine if changes are needed and to make any necessary changes. This review process is referred to
as the "triennial review." Public hearings were held to solicit comments for the 2000-2003 Triennial Review of surface
water quality standards during the summer of 2002. The Hearing Officer's recommendations were presented before
the state's Environmental Management Commission on October 10,2002.
The Hearing Officer's recommended changes included modifications to the existing standards for Arsenic,Cyanide,
Methylene Blue Active Substances (MBAS) and Total Residual Chlorine (TRC). The Hearing Officer also
recommended the addition of a standard for Methyl Tert-Butyl Ether (MTBE).
All of the Hearing Officer's proposed changes were adopted with the exception of MTBE. The changes took effect
on April 1,2003. This memo documents the changes to the NPDES permitting policy for TRC limits.
Total Residual Chlorine (TRC)
Previously,TRC had an Action Level Standard,except in designated Trout waters. The Triennial Review changes
modified the TRC Action Level Standard to an Aquatic Life Standard of 17µg/L. The Aquatic Life Standard will
remain for designated Trout waters.
Aquatic Life Standard— 17 µg/L (freshwater only) Trout Waters— 17 µg/L
Revised TRC Policy
> TRC limits will be assigned to permit renewals and all new permits issued after April 1, 2003.
' Facilities that do not use chlorine will not receive a TRC limit;however,the presence of a chlorine back-up
system to augment IJV or other disinfection requires a TRC limit.
Facilities discharging to streams with a 7Q10 <0.05 cfs (zero-flow streams) il.
receive a limit of 17 ug L.
> Limits will be capped at 28µg/L to protect against acute impacts.
Itis likely that many facilities will need to add dechlorination (or use another means of disinfection) to comply with
the new TRC standard. Therefore,a compliance schedule of 18 months from the effective date of the permit may
be added to any TRC limit added during the current permit renewal. This time period is allowed in order for the
facility to budget and design/construct the dechlorination and/or alternative disinfection system(s). The NPDES
Unit has promulgated minimum design criteria in order to expedite permitting of dechlorination equipment.

Sludge Management Plan
Narrative
The Process begins at the Water Plant Decant Basin where the supernatant is pumped off
the basin and sent to South Deep Creek. The alum sludge is gravity fed to two(2)drying
beds on site. The alum sludge is allowed to dry and the leachate is recirculated back
through the decant basin. The dry solids are then taken to the Yadkin County Landfill.
The remainder of the sludge is taken to the Yadkinville WWTP via Synagro solid
waste recyclers and put onto two(2)drying beds. The sludge is allowed to dry and the
leachate is put into the digester.The dry solids are the taken to the Yadkin County
Landfill.

The operation of the Decant Basin begins with the introduction of filter wash water and
alum sludge from the Water Plant. The sludge is settled and the supernatant is drawn off
and discharged into the South Deep Creek just below the Water Plant's Raw Water
Intake. The flow is determined by measuring the water level in the decant basin before
discharging and then measuring after emptying the decant basin. The difference is then
multiplied by 9400 gallons per foot. The discharge pumps are manually operated so that
no flow leaves the basin unless it is measured. The alum sludge is gravity fed to two (2)
drying beds discussed in the sludge management plan. Total suspended Solids are
measured two(2)times per month. Iron(Fe) and Aluminum(Al)are measured weekly as
are Total Chlorine and Suspended Solids. Upstream and Downstream Turbidity is
measured weekly.

attached NPDES discharge permit. This permit is issued pursuant to the requirements of North
Carolina General Statute 143-215.1 and the Memorandum of Agreement between North Carolina and
the U.S. Environmental Protection Agency dated December 6, 1983, and as subsequently amended.
If any parts, measurement frequencies or sampling requirements contained in this permit are
unacceptable to you, you have the right to an adjudicatory hearing upon written request within thirty
(30) days following receipt of this letter. This request must be in the form of a written petition,
conforming to Chapter 150B of the North Carolina General Statutes, and filed with the Office of
Administrative Hearings, Post Office Drawer 27447, Raleigh, North Carolina 27611-7447. Unless such
demand is made, this decision shall be final and binding.
Please note that this permit is not transferable except after notice to the Division. The Division
may require modification or revocation and reissuance of the permit.
This permit does not affect the legal requirements to obtain other permits which may be
required by the Division of Water Quality or permits required by the Division of Land Resources, the
Coastal Area Management Act or any other Federal or Local governmental permit that may be
required.

WLA Summary
Review of the Yadkin-Pee Dee River Basin WQMP that subbasin contains no impaired
waters, but most of the streams are threatened. Specifically, field investigation indicate
that South Deep Creek is impacted by sedimentation. Standard WTP monitoring
requirements (see C. Sullins memo 9/30/92) will remain in permit.
The stream classification manual indicates that the WTP intake is 0.1 mile downstream
Highway 601. However,Corey Basinger(WSRO) indicated that although the discharge
point is downstream of Highway 601,the intake is actually upstream of Highway 601.
Because the WTP is downstream of Highway 601,those preparing information for the
stream classification manual may have believed the intake was also downstream of
Highway 601. Therefore,the WTP discharges downstream of the intake into WS-IV
waters.
