The above project qualifies for a Nationwide Permit 3 and 12 authorization. NCDOT is authorized to replace the
existing 90 ft by 29.3 ft Bridge # 2 over Muddy Creek On NCSR 1321 with a 90 ft by 30 ft bridge and to replace associated
utilities. The project is located approximately 1.5 miles east of the intersection of NCSR 1321 and NCSR 1320 east of
Newby Landing in Perquimans County, North Carolina. The project will impact 0.05 acres of jurisdictional wetlands, All
terms and conditions for NWP #3 and 12 apply. The following Special Conditions apply:

1. In order to protect anadromous fishery resources, no in water work may occur between February 15 and June 30 of
any year.

2. All utility lines must be located a minimum of 10 feet below the stream substrate as shown on the submitted plans.

In my request letter for the replacement of Perquimans Bridge #2, dated September 16, 2014, I neglected to request a
NWP 12 and corresponding 401 Water Quality Certification. This project will impact jurisdictional wetlands, and a NWP
12 will be required.

Please revise the original request to include a NWP12 and 401 Certification. The submitted permit drawings include the
impacts proposed.
