Dear Mr. Robertson:
Ron Boone, Seren Homer, Jesse Barnes, and Kristin Potwora, of the NC Division of Water
Resources (DWR), Winston-Salem Regional Office, met with you, current owner of 1485 Double
Creek Drive in Lewisville, NC, to perform a compliance evaluation inspection of the home's single
family wastewater treatment system, on February 8, 2023. The inspection findings are summarized
below and detailed in the attached inspection report.
- You have lived in the house since June, 2020. You currently have no records of the system.
- You have not had the septic tank pumped out yet.
- You are placing chlorine tablets in the chlorinator.
- The outfall pipe was located and no issues were noted.
- You have not had any laboratory analyses of the effluent performed yet.
Please be aware that the Division may take enforcement action against any permittee who fails to
comply with any requirement of their permit. Pursuant to NC General Statute (NCGS) 143-215.6A,
a civil penalty of not more than twenty-five thousand dollars ($25,000.00) may be assessed against
any person who violates or fails to act in accordance with the terms, conditions, or requirements
of any permit issued pursuant to G.S. 143-215.1.

Dear Mr. Robertson,
Ron Boone, Seren Homer, Jesse Barnes, and Kristin Potwora, of the NC Division of Water Resources
(DWR), Winston-Salem Regional Office, met with you, current owner of 1485 Double Creek Drive in
Lewisville, NC, to perform a compliance evaluation inspection of the home's single family wastewater
treatment system, on February 8, 2023. The inspection findings are summarized below and in the
checklist on the following pages.
- You are the current owner of the residence but are not the current permittee. Mr. Boone provided
you a Name/Ownership Change form to fill out and send to the central office in Raleigh. Be sure to
follow all instructions provided on the form.
- In accordance with the NCG550000 permit, you must have the septic tank pumped out completely at
least every 5 years, or when the solids level in the tank reaches 1 /3 or more of the liquid depth of the
tank. You are aware of the approximate location of the tank but asked if we had any drawings of the
system. I have included a schematic of the system that was in our file for the residence.
Unfortunately, the schematic does not include measurements. There were no obvious signs of tank
malfunction or problems.
- There were no signs of malfunction or problems with the subsurface sand filter, however, the filter's
exact location is unknown.
- There were tablets in the chlorinator at the time of the inspection and you are using the proper
tablets (i.e. main ingredient is calcium hypochlorite). You should probably put no more than five
tablets at a time in the chlorinator. Once you better determine an accurate usage rate, you should
place no more tablets in the tube than would be used up over a month's time. Calcium hypochlorite
and a very potent oxidizer and can cause severe irritation of skin, eyes, etc. Be sure to wear eye,
respiratory, and hand protection when handling the tablets. Do NOT breathe fumes from the tablets.
Thoroughly rinse any body parts that contact the tablets with fresh water.
- The outfall pipe into the stream was located. It is readily accessible, and no problems or issues were
noted.
- No laboratory analyses of the effluent have been completed yet. We provided you a copy of the
general permit and went through its primary requirements with you. You may find certified laboratories
capable of running the required tests for you at this website:

We believe the Pace Labs office in Kernersville is the closest one to your residence.
- As you may already know, there is a $60 annual fee for this permit. It will be sent to you every year.
Once you fill out and submit the Name/Ownership Change form, our central office personnel can
change the permit to be in your name and all future correspondence will be sent to you, rather than
the former owner. In addition, the permit will be renewed every five years. You will need to do nothing
at this time. You will simply receive a copy of the renewed permit once it is completed.
